摘 要:目的探讨机体在不同疾病不同阶段体内主要内源性活性物质变化规律。方法应用发光免疫法分别检测30例不同病情患者术后1d、3d、7d皮质醇、C-肽、FT3、生长激素及睾丸酮血浆含量并进行动态A-PACHE-Ⅲ评分。结果术后1d皮质醇、C-肽、FT3、生长激素、性激素血浆含量均值较基础值差异有显著性(P<0.05),术后3、7d皮质醇、C-肽、FT3、性激素血浆含量均值较基础值差异有显著性(P<0.05)。神经外科组与普通外科组比较差异无显著性(P>0.05)。APACHE-Ⅲ评分值与血浆皮质醇、C-肽水平变化在术后1d、3d有线性正相关,与FT3水平变化有线性负相关。结论皮质醇、C-肽、FT3、生长激素、性激素在非内分泌疾病应激情况下均较基础状态时的情况有相应变化,其中皮质醇、C-肽、FT3变化在疾病早期与病情严重程度密切相关,与疾病的种类无关,而生长激素和性激素在疾病不同阶段的变化个体差异性较大。[Objective] To explore the regular of the changes of the major endogenous active peptides in different stages and disease. [Methods] The hydrocortisone, C-peptide, growth hormone, free tri-iodothyronine(FF3), growth hormone and testosterone levels were measured by electchemiluminescence immunoassay in 30 case of the different stages and disease patients on the basic state and 1st, 3rd, 7th postoperative day. APACHE m points were assigned to all patients in different stages. [Result] The hydrocortisone, C-peptide, growth hormone, FF3, growth hormone and testosterone levels on 1st postoperative day and the hydrocortisone, C-peptide, growth hormone and FT3 levels on 3rd, 7th postoperative day showed significant difference to the basic state(P〈0.05). The hydrocortisone, C-peptide, growth hormone, free tri-iodothyronine(FT3), growth hormone and testosterone levels in average surgery patients and nerve surgery patients showed no significant difference(P〉0.05). A significant positive relationship between the APACHE Ⅲ score and the levels of hydrocortisone,C-peptide on 1st and 3rd postoperative day was observed. A significant negative relationship between the APACHE Ⅲ score and the levels of FF3 on 1st and 3rd postoperative day was observed. [Conclusions] The hydrocortisone, C-peptide, growth hormone, FT3, growth hormone and testosterone levels obviously changed in stress state. The changes of the levels of hydrocortisone, C-peptide, and FT3 in earlier period of severe disease were correlated with the severity of disease and no relationship with the species of disease. The changes of growth hormone and testosterone in different stages could be different among individuals.
