摘 要:目的:探讨慢性粒细胞白血病(CML)起病和诊断中的多种预后相关因素。方法:选择CML患者58例,其中慢性期28例、加速期18例、急变期12例。对3组患者的原始细胞数、骨髓病理中p53、p63、CD34、Bcl-2、Mib和基因重排的检测进行比较。结果:3组CML患者的预后与起病时的原始细胞百分率、骨髓活检中的p53、p63、CD34、Bcl-2、Mib的免疫组化表达率、骨髓纤维化程度呈正比关系;3组患者的基因重排发现率不同,预后生存曲线亦完全不同。结论:CML的预后受到诸多因素的影响,骨髓中原始细胞的比例、骨髓病理中免疫组化的表达、骨髓纤维化程度和基因重排发现率是CML的一系列预后相关性指标。Objective:To explore the prognosis and related factors of chronic myeloid leukemia. Methods:Fifty-eight initial CML patients were enrolled in this study. Among them, 28 cases were in chronic phase, 18 cases were in accelerated phase and 12 cases were in blastic crisis. The blast cells of CML, the expression rate of p53, p63, CD34, Bcl-2, Mib, gene rearrangement in bone marrow and fibrosis was observed. Results:The results showed that the prognosis of newly diagnosed CML relationship with the blast cells, the expression rate of p53, p63, CD34, Bcl-2, Mib, gene rearrangement and fibrosis in bone marrow, Different evaluated and different survival could be obverved in each clinical phase (chronic, accelerated and blastic phase) of CML. The statistically significance was reached. Conclusion:The onset and the progression of CML and multi-factor influenced. Among them, the proliferative blast, immunohistochemistry expression of p53, p63, CD34, Bcl-2 and fibrosis, gene rearrangement is related with survival.
