摘 要:目的:探讨激素替代疗法(HRT)对心血管的影响,为性激素补充疗法提供心脏和血管病理形态学变化依据。方法:以去势雌性大鼠作为雌激素缺乏模型,在喂食高脂饲料情况下,行HRT后检测大鼠血脂及光镜下观察冠状动脉血管病理形态学变化。雌性大鼠分为6组,分别为A假手术组、B模型对照组、C尼尔雌醇组、D利维爱组、E倍美力组、F尼尔雌醇+孕酮组。结果:C、F组TG增高,与A、B、D、E组比较P<0.05;B组及C、E、F用药组TC增高,与实验前各组均值对照比较P<0.01,与A组比较P<0.05,C、E、F用药组与B组比较P<0.05;各组间HDL-C比较P<0.05;B组及C、E、F用药组LDL-C不同程度增高,与A组比较P<0.05;D组TC、LDL-C增高不明显,与A组比较P>0.05。实验各组大鼠心脏各级冠状动脉血管均见不同程度早期动脉粥样硬化症病变,F组病变程度最轻,A组、D组病变程度较轻,B组、C组、E组病变程度较重。结论:大鼠去势后HRT对血脂影响及冠状动脉粥样硬化病变的影响与雌激素类型、激素配伍使用及动物种属有关。Objective:To discuss the effect of hormone replacement therapy on cardiovascular system for providing the pathomorphological references for HRT on heart and blood vessels. Methods:Ovariectomized rats with high-fat diet were divided into 6 groups: shamoperation group (group A), model group(group B), nylestriol group(group C), livial group (group D), premarin group(group E), and nylestriol + Progesterone group(group F). We observed the pathomorphological change of coronary arteries under optical microseope, and the blood fat after HRT. Results:The levels of TG in the groups C and F were higher than those of the groups A, B, D, E ( P 〈 0.05) . The levels of TC in the groups B, C, E, and F were higher than the pre-experimental groups ( P 〈 0.01 ) and the group A ( P 〈 0.05 ) . Among the four positive groups, there were not differences between the groups C, E and F and the group B (P〉 0.05}. The level of HDL-C in each group is not significantly different ( P 〉 0.05). The levels of LDL-C in the groups B, C, E and F were higher than group A (P 〈0.05). The levels of Te and LDL-C in the group D changed indistinetively compared with the group A (P 〉 0.05) . The AS pathological changes on the early stage of hearts and coronary arteries were found in each group. The changes in the group F was least, in the groups A and D were minor, in the groups B, C and E were serious. Conclusions:The effect of HRT on blood fat and coronary atherosclerosis in ovarieetomized rats is related with the type of estrogen, the compatibility and the animal genus.
