胰岛素样生长因子Ⅱ在胃良恶性组织中的表达及意义

摘　　要：[目的]探讨胰岛素样生长因子Ⅱ(IGF-Ⅱ)表达与胃癌浸润、转移及TNM分期等生物学行为的关系。[方法]应用免疫组织化学SP法检测105例胃癌、60例正常胃黏膜和43例慢性萎缩性胃炎伴重度异型增生(CAGD)组织中IGF-Ⅱ表达。[结果]胃癌组IGF-Ⅱ阳性表达率81.0%(85/105),较CAGD组及正常胃黏膜组明显增高(P﹤0.01,P﹤0.01),且CAGD组较正常胃黏膜组有统计学差异(P﹤0.01)。IGF-Ⅱ的表达与胃癌的分化程度、浸润深度、淋巴结转移及TNM分期均密切相关(P﹤0.01)。[结论]IGF-Ⅱ过表达与胃癌发生有关,IGF-Ⅱ表达水平与胃癌细胞的侵袭深度、分化程度、淋巴结转移及TNM分期呈正相关,在胃癌的侵袭生长中可能发挥着重要作用,并影响其生物学行为。[Objective] To investigate the relationship of the expression of insulin growth factor- Ⅱ （IGF-Ⅱ） and histological type, TNM stage, infiltration as well as metastasis of gastric carcinoma （GC）. [Methods] The expression of IGF-Ⅱ were tested by S-P immtmohistochemistry in 105 cases with GC. 60 cases with normal gastric mucosa and 43 cases with chronic atrophic gastritis dysplasia （CAGD）. [ Results ] The positive expression rate of IGF- Ⅱ in GC group was 78.1% （82/105）, which was much higher than in CAGD and normal gastric mucosa groups （P 〈 0.01. P 〈 0.01 ）. There was significant difference between CAGD group and normal gastritis mucosa （P 〈 0.01 ）. The expression of IGF- Ⅱ was closely correlated to tumor differentiation, depth of infiltration, tumor size, lymph node metastasis and TNM stage （P 〈 0.01 ）. [Conclusion] The excessive expression of IGF- Ⅱ is closely correlated to carcinogenesis of gastric carcinoma （GC）. The expression of IGF- Ⅱ is positive correlated to the tumor differentiation, depth of infiltration, tumor size. lymph node metastasis and TNM stage. It may be an important role in the development of GC. It may also have an influence on its biological behavior.
