摘 要:目的建立卵清蛋白诱导的大鼠分泌性中耳炎动物模型,观察其病理改变,为探讨其发病机制奠定基础。方法20只健康的雄性SD大鼠,随机分为对照组(8只16耳,A组);OME组Ⅰ(8只16耳,B组)和OME组Ⅱ(4只8耳,C组)。OME组Ⅰ和OME组Ⅱ采用卵清蛋白腹腔致敏后分别经鼓膜穿刺和经听泡钻孔注射,耳内激发2次制成分泌性中耳炎模型;对照组以生理盐水替代卵清蛋白进行腹腔致敏和耳内激发。在末次耳内激发后2天处死动物,采用HE染色观察各组大鼠中耳黏膜病理变化及炎症细胞的改变,免疫组化染色检测中耳黏膜和骨髓腔中IL-4、IL-5的表达。结果A组1只、B组2只麻醉意外死亡。OME模型组I和组II大鼠听泡内均出现少量琥珀色积液,咽鼓管鼓室段纤毛上皮肿胀,排列紊乱,部分纤毛脱落,中耳黏膜增厚,黏膜下层、骨髓腔中有较多嗜酸粒细胞、淋巴细胞、中性粒细胞浸润,咽鼓管周围组织中肥大细胞增多、脱颗粒。对照组未见明显异常改变。两个OME模型组大鼠中耳粘膜和骨髓腔中IL-4、IL-5表达均明显高于对照组(P>0.05),但此两组间IL-4、IL-5阳性细胞数量差异无统计学意义。结论通过Ⅰ型变态反应能够成功诱发大鼠急性OME。经鼓膜或和经听泡钻孔注射两种方法的造模结果无差异。Objective To establish an animal model of otitis media with effusion induced by ovalbumin. Methods Twenty Sprague--Dawley (SD) rats were randomly divided into three group: group A ( control group, n= 16 ears) ,group B ( OMEgroupⅠ ,n=16 ears) and groupC (OMEgroupⅡ ,n=8 ears) . The rat model of OMEwas established by sensitizing with ovalbumin and later challenging in tympanic bullae. Ovalbumin was injected in to the animals of group B and C through tympanic membranes and by drilling tympanic bullae, respectively. The morphologic changes of the middle ear epithelial cells and inflammatory cells infiltration were observed under light microscope. The expression of IL--4, IL-- 5 in temporal bone marrow smears and middle ear mucosa was analyzed by immunohistochemistry (ISH). Results Three rats of Group A and B died as a result of the overdose of anesthetic. The erythena of the tympanic membrane, the effusion and infiltration of eosinophils, the mast cells and neutrophils were noted in the OME Group I and II while the control groups showed little change. As compared to the group A , the expression of IL-4 and IL-5 in both of OME model groups were remarkably stronger (both P〈0.05), while no significant difference was found between group B and group C (P〉0.05). Conclusion No significant differences are noted between the two injecting methods used, that is, via the tympanic membrane and drilling tympanic bullae. This suggests that to inject ovalbumin into the tympanic bullae should be a proper way in a rat model of otitis media with effusion.
