摘 要:目的:研究细胞周期调控因子p16、p27和CyclinD1在分化型甲状腺癌中的蛋白表达情况,及其在分化型甲状腺癌发生发展中的作用。方法:应用免疫组化S-P法对40例分化型甲状腺癌包括甲状腺乳头状癌和滤泡癌以及30例甲状腺腺瘤组织中p16、p27和CyclinD1的蛋白表达情况进行检测。结果:p16、p27和Cy-clinD1三种蛋白在40例分化型甲状腺癌的阳性表达率分别为57.5%、70.0%和62.5%;p16蛋白的阳性表达量与分化型甲状腺癌的颈淋巴结转移之间具有相关性(P<0.05);p27蛋白的阳性表达量与分化型甲状腺癌的颈淋巴结转移和原发灶的大小及包膜侵犯之间具有相关性(P<0.05);CyclinD1蛋白的阳性表达量与分化型甲状腺癌的颈淋巴结转移和原发灶的包膜侵犯之间具有相关性(P<0.05);p16的阳性表达与CyclinD1的阳性表达之间具有相关性(P<0.05),且呈负相关关系(rs=-0.283);p27的阳性表达与CyclinD1的阳性表达之间具有相关性(P<0.05),且呈负相关关系(rs=-0.279)。结论:p16、p27和CyclinD1三种蛋白的异常表达共同参与了分化型甲状腺癌的发生发展,可能成为判断分化型甲状腺癌恶性程度、转移及预后的指标。Objective:To investigate the expression of p16,p27 and CyclinD1 in differentiated thyroid cancer tissues ,and the role in the carcinogenesis of the thyroid cancer. Methods: To detect the expression of p16 ,p27 and CyclinD1 in 40 differentiated thyroid cancer including papillary thyroid carcinoma and follicular carcinoma and 30 benign tumor tissues including thyroid adenoma and nodular goiter by immunohistochemical S - P two - step method. Results: The positive expression rates of p16 ,p27 and CyclinD1 were 57.5% ,70.0% and 62.5% respectively in 40 differentiated thyroid thyroid cancer tissues, and were 20. 0%, 93.3% and 33.3% respectively in 30 benign tumor controls( P 〈 0.05 ). The positive expression of p16 was associated with cervical lymph node metastasis( P 〈 0.05 ) , but not associated with sex, age ,tumor size and primary focal extrathyroidal extension(P 〉0.05). The positive expression of p27 was associated with cervical lymph node metastasis and primary focal extrathyroidal extension and tumor size ( P 〈 0.05 ), but not associated with sex, age ( P 〉 0.05 ). The positive expression of CyclinD1 was associated with cervical lymph node metastasis and primary focal extrathyroidal extension ( P 〉 0. 05 ), There was negative correlation between the p16 and CyclinD1 ( rs = - 0. 283 ) , and was negative correlation between p27 and CyclinD1 (rs = -0. 279). Conclusion:The abnormal expression of p16,p27 and CyclinD1 may have an important role in the carcinogenesis of the thyroid, and they probably have a role in malignant progression of the thyroid cancer.
