摘　　要：目的:探讨食管鳞癌组织中p53蛋白、血小板反应蛋白-1(thrombospondin-1,TSP-1)的表达特征及临床意义。方法:利用免疫组织化学(SP法)法检测食管癌病人根治术切除标本的68例食管鳞癌组织中p53、TSP-1的表达状况及计数肿瘤内微血管密度(microvessel density,MVD),并对上述指标与临床病理特征、复发转移率之间的关系进行分析。结果:食管鳞癌组织中p53、TSP-1的总阳性率分别为72.06%和66.18%,p53表达与TSP-1表达呈负相关(P<0.01),TSP-1蛋白阳性表达组与TSP-1蛋白阴性表达组的MVD值分别为(18.37±4.86),(29.80±6.35),两组MVD值之间差异显著具有统计学意义(P<0.01);TSP-1与MVD呈负相关(P<0.01);p53的表达与病理分级、淋巴结转移和临床分期具有相关性。结论:TSP-1的阳性表达有可能通过抑制食管癌新生血管的形成,从而阻止浸润和转移。p53、TSP-1有可能成为判断食管癌恶性潜能和预后的重要生物学指标。Objective ：To investigate the expression of p53 protein and thrombospondin - 1 （ TSP - 1 ） in esophageal squamous carcinoma and their clinical significance. Methods： Using immunohistocbemical technique, to examine p53, TSP - 1 and MVD expression in 68 cases of esophagus squamous carcinoma. Results ： The positive rates of p53 protein and TSP - 1 in esophageal squamous carcinoma tissue were 72.06% and 66.18% respectively. The expression of p53 protein had a negative correlation with TSP - 1 （ P 〈 0.01 ）. MVD in TSP - 1 positive group （ 29.80 ± 6.35）, MVD in TSP - 1 negative group （ 29. 80 ± 6. 35 ） （ P 〈 0. 01 ）, TSP - 1 had a negative correlation with MVD（ P 〈 0.01 ）. The expression of p53 had a significantly correlation with pathological grading, lymph node metastasis and clinical staging. Conclusion： TSP - 1 may inhibit the angiogenesis, and prevent from the vessels development and metastasis of esophagus squamous carcinoma. Both p53 and TSP - 1 expression may be helpful to the understanding of mechanism of angiogenesis in tumor, p53 and TSP - 1 can be considered as biological indicators of malignant potential and prognosis of esophageal carcinoma.
