颈部吻合器在147例食管癌手术中的应用

摘　　要：目的探讨颈部吻合器应用于食管癌根治,胃食管颈部吻合口重建。方法用颈部吻合器对147例食管癌患者行胃食管颈部吻合术。重要手术步骤:胃食管的游离,颈部吻合器的应用及注意事项,胃及食管胃吻合口置于食管床。结果147例均一次吻合成功,术后无颈部吻合口瘘、出血、喉返神经损伤、乳糜胸发生。术后随访1～24个月,失访26例,死亡18例。结论颈部吻合器应用于食管胃吻合性能可靠,操作容易,手术时间缩短,并发症少,增加了颈部吻合的标准化,值得推广应用。Objective To evaluate the efficacy of circular mechanical stapler in cervical esophagogastric anastomosis after esophagectomy. Methods One hundred and forty - seven esophageal cancer patients underwent cervical esophagogastric anastomosis using the circular stapler. The key procedures of this operation included the liberation of stomach and esophageal, the cruces of using the circular mechanical stapler in the neck, and the stomach and stoma put in bed of esophageal. Results All of the cases were successful by once, no anastomotic leak, anastomotic blood, laryngeal nerve injury or chylothorax occurred. The follow - up time was 1 - 24 months, 26 cases lost follow - up, 18 cases died. Conclusion The capability of the circular mechanical stapler is credibility in cervical esophagogastric anastomosis, the operation is easy, and it can decrease the rate of complication. Also it can increase the standardization of cervical anastomasis, so it is deserving of apply.
