血浆脂联素水平与缺血性脑卒中亚型的关系

摘　　要：目的:研究血浆脂联素水平与缺血性脑卒中亚型的关系。方法:根据临床表现、影像学和实验室辅助检查结果,将106例急性脑梗死患者(病例组)分为动脉粥样硬化性脑梗死组(n=54)、心源性脑栓塞组(n=23)和腔隙性脑梗死组(n=29),同时选取同期该院门诊体检正常的健康者50例作为对照组。比较各组间脂联素水平及危险因素,包括代谢综合征。结果:(1)病例组患者血浆脂联素水平明显低于对照组[(5.7±1.8)mg/Lvs(8.8±2.4)mg/L,P<0.05]。(2)卒中亚型不同,脂联素水平不同,心源性脑栓塞组水平最高而动脉粥样硬化性脑梗死组水平最低[(8.0±2.2)mg/Lvs(5.5±1.6)mg/L,P<0.05],腔隙性脑梗死组居中[(6.7±1.4)mg/L]。(3)血浆脂联素水平与脑血管病危险因素相关。结论:动脉粥样硬化性脑梗死血浆脂联素水平较其他缺血性脑卒中亚型低。Objective To investigate the relationship between levels of plasma adiponectin and ischemic stroke subtypes. Methods According to clinical manifestations, medical imaging, and laboratory tests, 106 consecutive patients with acute cerebral infarctions were categorized to atherosclerotic infarction group （n = 54）, cardioembolic infarction group （n = 23）, and lacunar infarct group （n = 29）. The risk factors for infarction and plasma adiponectin levels were compared among the three groups. Results Plasma adiponectin levels were significantly lower in patients with cerebral infarctions than in normal controls [ （5.7± 1.8） mg/L vs （8.8 ±2.4） mg/L, P 〈 0.05 ]and differed in diverse stroke subtypes, being highest in cardioembolic infarction group, lowest in atherosclerotic infarction group [ （8.0± 2.2） mg/L vs （5.5 ±1.6） mg/L, P 〈 0.05 ], and intermediate in lacunar infarct group[ （6.7 ± 1.4） mg/L]. Levels of plasma adiponectin correlated with the risk factors for cerebrovascular disease. Conclusion Plasma adiponectin levels are lower in atherosclerotic cerebral infarction than in the other subtypes of ischemic stroke.
