结肠癌中P16、PCNA基因表达及AgNOR定量分析的临床相关性研究

摘　　要：目的探讨结肠癌中P16、PCNA基因表达及AgNOR定量分析与肿瘤的临床特征和预后的关系。方法用免疫组化技术检测58例结肠癌组织中的P16、PCNA基因表达及AgNOR定量分析研究。结果1)结肠癌中P16阳性表达率为55.1%(32/58),主要分布于分化好或无转移的肿瘤,而低分化或转移组病例P16表达多呈阴性。2)PCNA表达与结肠癌组织学分级、转移及生存率有关。低分化、转移的、生存期≤3年者的PCNA阳性表达率均分别高于高分化、无转移的、生存期>3年者且差异有高度显著性。3)AgNOR颗粒数与总面积随分化的程度增高而减少,与三年生存率呈负相关。结论P16、PCNA基因表达及AgNOR定量分析与多项结肠癌的临床特征和预后上的有关,说明联合检测以上定性、定量指标,将更加有效地判断结肠癌的生物学行为和病人的预后。Objective To explore the relation of P16, PCNA expression and AgNOR image analysis in the genesis and progression of colon carcinoma. Methods The expression P16, PCNA and Ag- NOR count or area in 58 cases of colon carcinoma were quantitatively studied by immunohistochemical staining technique and image analysis system. Results 1） The positive expression of P16 was 55. 1%. The major of high different citation grading and metastatic group. 2） The PCNA expression correlated with the clinical stage, differentiation grading and metastatic. The PCNA expression of the low differentiation grading, metastatic and survival stage≤3 years were higher than those of high differentiation grading, original tumors and survival stage〉3 years group. 3） The results of AgNOR count or area image analysis in patients were also related to the 3 years survival rate. Conclusions It suggests that immuno--staining of P16, PCNA expression and AgNOR image analysis are useful in evaluating the malignant degree of colon carcinoma and it may guide to make therapeutic decision.
