冠根斜折牙体重塑后桩核冠修复对抗折力的影响

摘　　要：目的比较三种冠根斜折牙经牙体重塑与不重塑,桩核冠修复后的抗折强度及牙折形式。方法对42颗近期拔除的形态、大小相似、根管治疗后的完整的人上颌中切牙,截冠后随机分为七组:唇舌向、舌唇向、近远中向冠根斜折经牙体重塑及不重塑组、无斜折对照组。经铸造桩核及铸造全冠恢复外形,自凝塑料包埋后,固定于生物力学试验机上测试实验牙的抗折强度并观察其抗折类型,应用SPSS分析软件进行统计学分析。结果冠根斜折牙体重塑后桩核冠修复的抗折强度高于不重塑者;近远中向斜折重塑与不重塑组二者无统计学差异;不重塑的冠根唇向斜折牙的抗折强度明显高于舌向斜折者。结论牙体重塑能提高冠根斜折牙的抗折强度;牙折形式与牙体重塑与否无关。Objective To compare the fracture resistance and fracture pattern of the teeth after post - and - core crown restoration, which had suffered from three types of crown and root oblique fractures and treated with or without tooth reconstruction respectively. Methods Forty - two recently extracted, sound human maxillary incisors of similar size, after root canal therapy and removal of crowns, were divided into 7 groups randomly （ n = 6 ） ： Three experimental groups had labio - lingual, linguo - labial, or mesio - distal crown and root oblique fractures along with tooth structure reconstruction ; Other 3 experimental groups had labio - lingual, linguo - labial, or mesio - distal crown and root oblique fractures without tooth structure reconstruction ; the control group had no crown and root oblique fracture. All groups were restored with post - and - core crown and embedded in self - cure resin, and the fracture resistance was examined by using a universal testing machine and the fracture patterns were observed. SPSS software was used for statistical analysis. Results The fracture resistance of the reconstructed teeth with post - and - core crown restoration was higher than that of unreconstructed one. While there was no statistical difference between the mesio - distal oblique fracture group with tooth reconstruction and that without tooth reconstruction. The fracture resistance of linguo - labial oblique fracture group without tooth reconstruction was obviously higher than that of the labio - lingual oblique fracture. Conclusion The reconstruction of the crown and root oblique fracture before the pest - and - core crown restoration is good for reinforcing the fracture resistance of the fractured teeth.
