脑卒中患者血清超敏C-反应蛋白、神经元烯醇化酶的变化

摘  要:目的:探讨脑卒中患者血清超敏C-反应蛋白(Hs-CRP),神经元特异性烯醇化酶(NSE)水平与脑卒中病灶大小和预后的关系。方法:收集脑卒中患者200例为病例组,选择200例性别、年龄和种族等与病例组相匹配的无脑卒中者作为对照组,用胶乳增强免疫比浊法检测血清Hs-CRP,并比较两组血清总胆固醇(TC),三酰甘油(TG)水平的差异。用固相酶联免疫法测定脑卒中患者住院时及住院第1天、3天、5天和15天时血清NSE浓度的动态变化。通过CT检查脑出血或梗死病灶体积。结果:脑卒中组Hs-CRP(6.25±0.25mg.L-1)高于对照组(2.42±0.18mg.L-1),差异具有统计学意义(P<0.01),脑卒中组TC(5.54±0.72mmol.L-1)高于对照组(4.42±0.68mmol.L-1),差异具有统计学意义(P<0.05),两组TG水平差异无统计学意义。脑梗死、脑出血组在住院时及住院第1天、3天和5天时的平均血清NSE水平均比对照组显著增高(P<0.01),NSE浓度和梗死出血体积以及神经功能缺损之间存在密切关系(P<0.01),但在第15天脑卒中组血清NSE水平与对照组比较差异无统计学意义。结论:Hs-CRP和NSE水平变化可能是判断脑损伤和评价预后的一个敏感指标。Objective:To explore the relationship of serum levels of high-sensitivity C-reactive protein(Hs-CRP),neuron-specific enolase(NSE) in predicting extent of disease and clinical prognosis of functional outcome after stroke.Methods:200 patients with stroke were included as the case group in this study while 200 subjects without stroke matched with the case group for sex and age were collected as the control group.The high-sensitivity C-reactive protein(Hs-CRP) was measured by latex-enhanced turbidometry.The serum lipids profile were compared in the two groups.The dynamics of change of serum NSE concentration was measured by solid-phase enzyme immunoassay in 200 patients with stroke between on admission and on days 1,3,5 and 15.The volume of cerebral infarction and cerebral hemorrhage was measured by computed tomography.Results:The serum concentration of Hs-CRP was higher in the patients than that in the control group(6.25±0.25 mg/L versus 2.42±0.18 mg/L),P〈 0.01.The serum total cholesterol concentration were higher in the patients than that in the control group(5.54±0.72 mmol/L vs.4.42±0.68 mmol/L),P〈0.05,but there was no difference in triglycerides concentration between two groups.Mean serum NSE levels were significantly higher among patients with cerebral infarction and cerebral hemorrhage than controls(P〈0.01) on admission and on days 1,3,5,a significant correlation was found among NSE concentration in serum and the volume of cerebral infarction and cerebral hemorrhage and the degree of neurological and functional deficit(P〈0.01),but the NSE levels were not significantly different at fifteenth day.Conclusion:The changes of serum Hs-CRP and NSE levels might be a sensitive index to judge brain damage degree and prognosis prediction.
