摘  要:目的:探讨上皮性卵巢癌患者血VEGF水平与癌组织淋巴管和血管生成及卵巢上皮性肿瘤发展的关系。方法:采用酶联免疫吸附试验双抗体夹心法(ELISA)法检测20例上皮性卵巢肿瘤、12例交界性上皮性卵巢肿瘤和48例上皮性卵巢癌患者血中VEGF水平,同时以CD34、VEGF-3分别标记肿瘤微血管内皮细胞、微淋巴管内皮细胞,应用免疫组织化学方法检测肿瘤组织中微血管(MVD)、微淋巴管(LVD)的生成密度。结果:良性、交界性及恶性卵巢上皮性肿瘤患者血VEGF水平依次升高,分别为(686.28±149.68)ng/L、(750.28±90.12)ng/L和(1183.23±293.22)ng/L,P<0.001,肿瘤组织中微血管(MVD)、微淋巴管(LVD)的生成密度依次升高,P<0.001;上皮性卵巢癌中患者血VEGF与组织中LVD、MVD表达正相关r=-0.351,P=0.015、r=0.625,P<0.001,并在浆液性卵巢腺癌、低分化卵巢腺癌中水平明显升高P=0.046、P=0.029,而与临床分期无关。结论:血VEGF水平与上皮性卵巢癌血管生成及淋巴管生成相关,可作为反映卵巢上皮性肿瘤生物学行为的指标之一,有助手判断卵巢癌的预后。Objective: To investigate the relationship of serum vascular endothelial growth factor with lymphangiogenesis (LVD) and angiogenesis (MVD) in epithelial ovarian neoplasm. Methods: Serum levels of VEGF in 48 samples of ovarian carcinoma, 12 samples of borderline ovarian tumor, 20 samples of benign ovarian tmnor was measured by Sandwich-LISA; LVD and MVD were counted by manu- al counting in tumor tissue. Results: The serum levels of VEGF were increased in turn in benign ovarian tumor, borderline ovarian tumor and ovarian carcinoma ( P 〈 0. 001 ) ; LVD and MVD were also increased in turn ( P 〈 0. 001 ) ; There were positive correlation between the serum levels of VEGF and LVD, MVD in epithelial ovarian neoplasm (r =0. 351, P =0. 015, r =0. 625, P 〈0. 001, and the serum levels of VEGF were higher in serous cystadenocarcinoma and in poor diffcerentiation ( P =0. 046, P=0. 029 respectively) . Conclusion: The serum levels of VEGF are correlated with LVD and MVD, and involved development of epithelial ovarian neoplasm. It could be served as one of markers reflecting tumor biological behavior, and assisted to estimate outcome.
