食管胃手工分层吻合与机械吻合的成本效果分析

摘　　要：目的从卫生经济学的角度探讨食管胃机械吻合与三叶钳食管胃分层吻合的治疗的成本和效果。方法290例患者接受食管癌根治术,根据食管胃吻合方式的不同,分为手工吻合组和机械吻合组,应用成本-效果分析法进行回顾性分析。结果食管癌采用三叶钳辅助食管胃分层吻合组医疗成本明显低于机械吻合组,而效果两组差异无显著性。结论食管癌采用三叶钳辅助食管胃分层吻合组医疗成本低,是减少一次性医疗器械的使用的有效方法之一,值得临床推广应用。Objective To investigate the therapeutic cost and effect of esophago - gastroanastomosis by mechanical inosculation or by delaminating inosculatinn with three leaf clamp from point view of medical economics. Methods 290 patients accepted radical operation for esophageal carcinoma were divided into hand - made inosculation group and mechanical inosculation group according to different styles of esophago - gastroanaslomosis, then the cost - effectiveness was retrospectively analyzed by using analytical method. Results The medical Cost for esophago - gastroanastomosis by delaminating inosculation with three leaf clamp was obviously lower than that of mechanical inesculation group, and there was no significant difference in therapeutic effect in both groups. Conclusion The medical therapeutic cost in group of esophago - gastroanastomosis by delaminating inesculation with three leaf clamp group is lower than that of group with mechanical inosculation for treatment of esophageal carcinoma. There is no significant difference between both groups.
