摘 要:目的研究妊娠滋养细胞肿瘤(GTT)中端粒酶催化组分(h-TERT)与增殖细胞核抗原(PCNA)的表达意义。方法采用免疫组化Evision法和S-P法分别检测了正常早期绒毛(NP)10例、完全性葡萄胎(CM)10例、侵袭性葡萄胎(IM)20例、绒癌(CCA)19例中h-TERT与PCNA的表达。结果h-TERT在NP、CM、IM、CCA中表达率分别为18.47%、5.68%、20.50%、32.84%,差异有统计学意义(P<0.01),其中CM与IM之间、CM与CCA之间、IM与CCA之间,差异均有统计学意义(P<0.01)。PCNA在NP、CM、IM和CCA中表达率分别为10.40%、20.76%、53.60%、51.95%,差异有统计学意义(P<0.01),其中NP与IM之间、NP与CCA之间、CM与IM之间、CM与CCA之间,差异均有统计学意义(P<0.01)。h-TERT与PCNA阳性率呈正相关(P<0.05)。结论端粒酶活性增高促进了GTT的恶性进展,h-TERT的检测有助于良恶性GTT的鉴别;恶性GTT中细胞增殖活性增高,PCNA的检测有助于良恶性GTT的鉴别;端粒酶活性增高促进了GTT的增殖活性。Objective To investigate the features of telomerase and PCNA in gestational trophoblastic tumor (GTT).Methods Telomerase catalytic protein subunit (h-TRT) and PCNA were detected immunohistochemically in paraffin-embedded tissue of 10 cases of normal placenta(NP), 10 cases of complete hydatidiform mole (CM), 20 cases of invasive hydatidiform mole (IM) and 19 cases of choriocareinoma(CCA). Results In NP, CM, IM and CCA, The h-TERT index(TI) was 18.47%, 5.68%, 20.50% and 32.84% respectively with significant differences among them ( P 〈 0.05) . There were significant differences between CM and IM ( P 〈 0.01 ), and between CM and CCA ( P 〈 0.01), and also between IM and CCA ( P 〈0.01). The PCNA index(H) was 10.40%, 20.76%, 53.60% and 51.95% respectively with significant differences among them ( P 〈 0.01). There were significant differences between NP and IM ( P 〈 0.01), and between NP and CCA ( P 〈 0.01 ), and between CM and IM ( P 〈 0.01 ), and also between CM and CCA ( P 〈 0.01 ). Significantly positive correlation was found between TI and PI ( P 〈 0.05 ). Conclusion The progress of GrIT are associated with increasing activity of telomerase, h-TERT can be helpful to distinguish IM and CCA from CM. The process of GrIT is associated with tmphoblastic cell overproliferation. PCNA is helpful to distinguish IM and CCA from CM. The increasing activity of telomerase is helpful to the cell proliferation in GTT.
