摘  要:目的以黑色素瘤抗原-A1和A3(MAGE-A1和MAGE-A3)基因mRNA为标记,检测乳腺癌患者循环肿瘤细胞,并探讨其临床意义。方法采集60例乳腺癌患者癌组织、癌旁组织标本及术前、化疗后外周血和60例非肿瘤患者外周血,用巢式RT-PCR检测癌组织、癌旁组织标本及外周血单核细胞(PBMC)中MAGE-A1和MAGE-A3基因mRNA。结果28.3%(17/60)和23.3%(14/60)的乳腺癌患者PBMC中分别检测到MAGE-A1和MAGE-A3基因的mRNA,35.0%(21/60)的乳腺癌患者PBMC中至少可以检测到1种MAGE基因mRNA。乳腺癌患者PBMC中MAGE-A1和MAGE-A3基因mRNA的检出率与肿瘤组织中MAGE的检出率相关(P<0.0005),肿瘤组织及外周血中MAGE的表达与肿瘤的分期、肿瘤的病理类型、年龄、受体状况、CerbB-2、Ki-67无相关性(P>0.05)。结论MAGE-A1和MAGE-A3基因mRNA可以作为特异性肿瘤标记物用于检测乳腺癌患者PBMC中的乳腺癌细胞,其检测结果可能有助于判断乳腺癌患者的预后。Objective To detect the circulating tumor cells of breast cancer patients by the MAGE-A1 and MAGE-A3 gene as specific marker and to discuss its clinical significance.Methods 60 patients with breast carcinoma were included in the study.The mRNA of the MAGE-A1 and MAGE-A3 gene was detected by nested RT-PCR in breast carcinoma,para-cancerous tissues,and peripheral blood.The mRNA of the MAGE-A1 and MAGE-A3 gene was also analyzed in the peripheral blood of 60 healthy controls.Results MAGE-A1 and MAGE-A3 mRNA were positive in 28.3%(17/60) and 23.3%(14/60) in the PBMC of patients with breast carcinoma..At least one MAGE gene mRNA was positive in 35.0%(21/60) of patients with breast cancer.The positive rate of MAGE-A1 and MAGE-A3 mRNA in the PBMC of the breast cancer patients was related to the MAGE expression in the tumor tissue.MAGE expression in the tumor tissue and peripheral blood was not related to stage,histological types,age,receptor status,CerbB-2 and Ki-67 expression.Conclusion MAGE-A1 and MAGE-A3 may be used as the specific tumor markers to detect the circulating breast cancer cells.Detection of MAGE-A1 and MAGE-A3 could be useful in the prognosis of the breast cancer.
