紫杉醇每周与标准3周给药联合卡铂治疗晚期非小细胞肺癌的临床观察

摘　　要：目的比较紫杉醇每周与标准3周给药联合卡铂治疗晚期非小细胞肺癌的疗效和不良反应。方法87例非小细胞肺癌患者被随机分成每周给药方案组(周疗组)和标准3周给药方案组(标准组)。周疗组,46例,紫杉醇每周给药80mg/m2第1、8、15天,卡铂(AUC=5),第2天;标准组,41例,紫杉醇每3周给药175mg/m2,第1天,卡铂(AUC=5)第2天。结果周疗组与标准组有效率无显著性差异(P>0.05)。周疗组血色素下降较标准组多,标准组肌肉关节稍较周疗组多,均有显著性差异(P<0.05)。其他不良反应相近。结论两组有效率相近,无显著性差异(P>0.05),周疗组良好的非血液毒性状况使得它成为对进展期NSCLC患者的一种可供选择的治疗方案。Objective To compare the efficacy and toxicities of weekly paclitaxel and standard 3-week regimen of paclitaxel with carboplatin in the treatment of patients with advanced non-small cell lung cancer.Methods 87 patients with NSCLC were randomly assigned to weekly paclitaxel arm（n=46）,and standard 3 week regime of paclitaxel（n=41）.The weekly paclitaxel arm included paclitaxel 80 mg/m^2 weekly on day 1,8 and 15,and carboplatin AUC=5 on day 2 for every 4 weeks.Standard 3-week arm included paclitaxel 175 mg/m^2 on day 1 with carboplatin AUC=5 on day 2 for every 3 weeks.Results The response rate of weekly regimen was not significantly different to that of standard 3 week regime（P〉0.05）.Significant more chemotherapy-related anemia was seen in the weekly paclitaxel group,where significantly more patients in the standard 3 week group had bone and muscle pain.Conclusion Carboplatin combined with weekly paclitaxel or standard 3 weeks paclitaxel has similar efficacy in the treatment of patients with NSCLC.The favorable nonhematological toxicities of weekly paclitaxel regimen make this an alternative treatment option for patients with advanced NSCLC.
