摘　　要：目的探讨子宫颈环形电切术（Leep）在诊断和治疗子宫颈上皮内瘤变Ⅲ（CINⅢ）中的作用。方法2003年7月-2005年6月阴道镜下宫颈活检病理诊断为CINⅢ的130例患者行Leep诊治并跟踪随访24个月，评Leep手术对CINⅢ的诊断和治疗作用。结果阴道镜下宫颈活检与Leep手术病理诊断相符的75例（57．7％），低于阴道镜下宫颈活检结果的36例（27．7％），高于阴道镜下宫颈活检结果的19例（14．6％）。追踪随访的111例患者中，比较Leep术前和术后1年、2年宫颈异常鳞状细胞和高危型人乳头瘤病毒感染率，P〈0．01，差异有显著性。2年治愈率达99.0％，复发率0．9％。结论Leep手术在CINⅢ诊断中可以弥补阴道镜下宫颈活检的不足，减少漏诊率，同时是治疗CINⅢ的一种行之有效的方法。Objectlve To discuss the clinical value of loop electrosurgical excision procedure （LEEP） in diagnosis and treatment of cervical intraepithelial neoplasia of grade Ⅲ（ CIN Ⅲ）. Methods There 130 outpatients with cervical intraepi-thelial neoplasia of grade Ⅲ diagnosed by colposcopic multiple biopsy were surgicallly treated with LEEP between July 2003 and June 2005. Results The LEEP pathological results were accordant with the pathological ones of colposcopic multiple biopsy in 75 cases （57.7%）. The cure rate was 99.0%, The recudescent rate was 0.9%, Conclusion LEEP is more reliable than colposcopic multiple biopsy in diagnosing CINⅢ,and is a safe and effective procedure for treatment of CINⅢ.
