睑下垂术后暴露性角膜炎的回顾性分析

摘　　要：目的:探讨睑下垂术后暴露性角膜炎的发生原因,并制定相应的预防措施。方法:对我院2001/2007收治的28例(30眼)各种术式睑下垂术后暴露性角膜炎的临床资料(包括一般资料,术前体征,手术方式,术后体征及治疗措施)进行回顾性分析。结果:术前患干眼症,术中医源性损伤,术后Bell氏现象阴性,患者因怕痛而不瞬目,睑裂闭合不全等都是重要的睑下垂术后暴露性角膜炎的致病因素。结论:术前全面细致的检查,术中对应的预防措施,术后及时正确的处置,和给患者克服疼痛的鼓励是防治暴露性角膜炎的关键点。AIM： To investigate the causes of exposure keratitis after blepharoptosis surgery and to establish proper preventive measures. METHODS： Clinical data of 28 cases （30 eyes） with exposure keratitis admitted to our hospital from 2001 to 2007 were retrospectively analyzed, including general information, signs before and after surgery, surgical procedures and remedies. RESULTS： The causes of exposure keratitis included dry eyes before surgery, being negative to Bell＇s phenomenon, iatrogenic injuries, no blinking due to pain and incomplete closure of eyelid fissure. CONCLUSION ： Comprehensive and meticulous physical examination before surgery, proper intraoperative preventive measures, careful and correct postoperative observations and remedies, and encouragement to patients are the key points to prevent exposure keratitis.
