立体定向放射配合卡莫氟治疗晚期胰腺癌近期疗效观察

摘　　要：目的观察全身伽玛刀配合卡莫氟治疗胰腺癌的临床效果。方法应用OUR-QCD体部伽玛刀治疗胰腺癌109例,根据胰腺病灶的三维形状确定靶点数目及准直器型号,采用50%～85%的等剂量曲线包绕靶区,总剂量35～40Gy,分割成10～12次。卡莫氟口服150～200mg,3次/d,连用4周。结果①治疗后CA19-9从（534.27±413.28）KU/L降至（343.15±265.31）KU/L,CEA从（20.73±55.16）ng/ml降至（11.58±23.33）ng/ml;②有效率：CR＋PR为71%（77/109）;③急性放射反应：主要的急性放射反应为消化系统和血液系统不良反应。结论伽玛刀对胰腺癌有较高的局部缓解率,卡莫氟在增敏方面效果显著。Objective To discuss the curative effect of whole body γ-knife and kamof therapy on advanced pancreatic cancer.Methods The OUR-QGD Whole bodyγ-knife was used treat advanced pancreatic cancer.According to the orientation shape of the pancreatic cancer,number of aims and the size of the collimator were comfired,the targets was covered with 50%～85% isodose curve.The total radio dosage was 35～40Gy,divided into 10～12 times.The commended dosage of kamof is 150～200mg third a day and continye four weeks.Results After the treatment,the mean value of CA 19-9 reduced from（534.27±413.28）KU/L to（343.15±265.31）KU/L,CEA from（20.73±55.16）ng/ml,to,（11.58±23.33）ng/ml.The availability CR＋PR 71%.The majoy toxicities were gastrointestinal tract reaction and the toxicities of hematology.Conclusion Whole body γ-knife has higher efficacy for pancreatic cancer.With regard to the significant protective effect,kamof marine is recommended.
