异种脱钙骨基质对拔牙创愈合的作用

摘　　要：目的:评价异种(牛)脱钙骨基质对拔牙创愈合及牙槽嵴形态改建的作用。方法:选用18只健康成年犬,拔除双侧下颌第四前磨牙,左侧拔牙创内充填异种脱钙骨基质作为实验组,右侧拔牙创不处理作为对照组,同体对照;拔牙后分别于第4、12、24周各处死6只动物,头颅标本行螺旋CT扫描,应用多平面重建技术测量拔牙创CT值及牙槽嵴吸收值;切取拔牙创组织制作HE染色切片行组织学观察。结果:各时间点实验组拔牙创CT值均显著高于对照组(P<0.05);4周时实验组牙槽嵴吸收值低于对照组,但二者之间的差异无统计学意义(P>0.05),第12、24周时实验组牙槽嵴吸收值均显著低于对照组(P<0.05);组织学观察可见:在4和12周时,实验组成骨情况优于对照组,24周时2组无显著差异。全过程实验组均未见明显炎细胞浸润。结论:异种脱钙骨基质可以促进拔牙创的愈合,保持牙槽嵴高度,有利于后期的牙种植或义齿修复。Objective.To assess the efficacy of decalcified heterogeneous（cattle） bone matrix （DBM） on the healing of dental sockets and alveolar bone rebuilding. Methods： 18 dogs＇bilateral mandibular fourth premolars were extracted, the test group（ left sockets）was filled with DBM, while the control group（ right sockets） was left without any treatment. 6 dogs were sacrificed at the end of the 4th, 12th and 24th week respectively. Dog heads were scanned by spiral CT. CT values of sockets and heights of the alveolar crest absorption were measured by multiplane reconstruction （MPR）technique. The new tissue formed in sockets were sectioned for histological observation. Resuits ：（1）At all time points, CT values of the sockets in the test group were significantly higher than those in the control group（ P 〈0.05） ; （2）At the end of the 4th week, heights of the alveolar crest absorption in the test group was less than that in the control group, but the difference between the two groups had no statistical significance（ P 〉 0.05 ）. At the end of the 12th and 24th week, heights of the alveolar crest absorption in the test group were significantly less than those in the control group（ P 〈 0.05 ） ; （3）At the end of the 4th and 12th week, the new bone formations in test group were better than those in control group. But at the end of the 24th week, there was no significant difference between the two groups. There was no significant inflammatory cell infiltrating during the whole process. Conclusion： Filling heterogeneous DBM in dental sockets can promote healing of sockets, preserve height of the alveolar crest, benefit for later dental implantation and prosthetic rehabilitation.
