尿液中DD3 mRNA检测对前列腺癌早期诊断意义的研究

摘　　要：目的:探讨二氢二醇脱氢酶3(DD3)在前列腺癌患者早期诊断和预后判断中的作用。方法:收集非前列腺疾病患者、前列腺良性增生和前列腺癌患者的尿液标本沉淀,采用逆转录-聚合酶链反应(RT-PCR)方法对其中的DD3 mRNA表达进行研究,应用SPSS13.0结合临床数据进行相关分析,并与前列腺特异性抗原(PSA)比较。结果:前列腺癌患者尿液标本中DD3 mRNA表达的相对含量是非前列腺疾病患者的4.5倍、前列腺良性增生患者的2.7倍;DD3mRNA相对含量值与Gleason评分均呈正相关,各个Gleason评分/分化程度中,DD3 mRNA相对含量值有明显差异,分化程度越低,DD3 mRNA相对含量值越高;用受试者工作特征曲线分析,各指标PSA、F/T和DD3 mRNA相对含量值的曲线下面积分别为0.744、0.788和0.921,DD3 mRNA相对含量值的检验效能最大。结论:尿液中DD3 mRNA的检测应用于前列腺良性增生和前列腺癌的鉴别时,可能要比PSA强,而且是一种预测前列腺癌预后的标志之一。OBJECTIVE： To approach the role of DD3 in early diagnosis and prognostic judgement of prostate cancer. METHODS.. The urine samples were collected, including nonprostate disease, benign prostatic hyperplasia and prostate cancer patients. The expression of DD3 mRNA was extracted by RT-polymerase chain reaction. SPSS 13.0 was used for data analysis connecting with the clinical data, moreover. PSA was compared with DD3 gray scale ratios. RESULTS： DD3 gray scale ratios of prostate cancer were 4.5 and 2. 7 times of non-prostate drsease and benign prostatic hyperplasia patients. DD3 gray scale ra tios were positively correlated with Gleason scores, and there was significant difference between Gleason score grades each other, and they became higher when the differentiation degree was higher. In the applied receive operative characteristic curve analysis data, the areas under the curve of PSA,F/T, and DD3 gray scale ratios were 0. 744,0. 788,0. 921, respectively. The efficacy of DD3 gray scale ratios was maximum. CONCLUSION.. The detection of DD3 mRNA in urine could be applied to discriminate prostate cancer and benign prostatic hyperplasia, and the diagnosis capability of it may be better than that of PSA, eventually, and it is a useful tumor marker which could guide the prognosis of prostate cancer.
