胃癌高发与低发区人群HLA-DPB1等位基因多态性的研究

摘　　要：目的:探讨中国北方胃癌高、低发区人群HLA-DPB1等位基因多态性分布与胃疾病及幽门螺杆菌(Helicobacter pylori,Hp)感染的关系。方法:采用聚合酶链反应-限制性片段长度多态(PCR-RFLP)技术分析863例研究对象的HLA-DPB1等位基因多态性;酶联免疫吸附方法(ELISA)检测血清Hp-IgG。结果:1)中国北方地区人群中检出4种DPB1等位基因多态:DPB1*1401(65%)最高,其次依次为*1701(5.4%)、*0901(4.0%)和*0301(0.9%)。2)胃癌高、低发人群DPB1*0901和*1701等位基因频率分布差异有统计学意义(*0901:χ2=36.145,P<0.01;*1701:χ2=35.526,P<0.01)。3)胃癌与非癌对照组间DPB1*0901、*1401、*1701等位基因频率分布差异无统计学意义;Hp是否感染组间DPB1*0901、*1401、*1701等位基因频率分布差异无统计学意义。结论:HLA-DPB1*0901、*1701等位基因多态存在胃癌高、低发区人群分布差异,未发现HLA-DPB1等位基因多态与胃癌易感性及Hp感染有关。OBJECTIVE： To investigate the relationship between HLA-DPB1 polymorphisms and gastric cancer and the infection of Hp in population at the high and low areas of gastric carcinoma in northern China. METHODS： PCR-RFLP was applied to analysis HLA-DPB1 polymorphisms in 863 cases and ELISA was used to determine Hp-IgG. RESULTS： 1 ） Four HLA-DPB1 polymorphisms were detected： The most predominant DPB1 allele was DPB1 ＊ 1401 with the frequency of （65%）, followed by DPB1 ＊ 1701（5.4%）, ＊ 0901（4.0%）, ＊ 0301（0.9%）; 2） ＊0901 and ＊ 1701 alleles were found significantly different in the population between the high and low areas of gastric carcinoma （＊ 0901：χ ^2=36.145, P〈0.01; 1701：χ^2=35.526, P=0.01）; 3）Between the no gastric cases and gastric carcinoma, no difference among DPB1 ＊ 0901, ＊ 1401 and ＊ 1701 was found; no significant difference was found among DPB1 ＊ 0901, ＊ 1401, ＊ 1701 alleles between Hp＋positive and Hp-negative population. CONCLUSIONS： HLA-DPB1 ＊ 0901 and ＊ 1701 have significant differences in the population between the high and low areas of gastric carcinoma, and no association was found between 0901,1401, ＊ 1701 and gastric cancer susceptibility and infection susceptibility of Hp.
