摘 要:目的:比较PET-CT、MRI与螺旋CT对肝癌病灶的检出差异,为肝癌放疗时确定靶区病灶积累影像学的比较资料。方法:回顾性分析诊断为原发性肝癌且治疗前进行了PET-CT、MRI与螺旋CT检查的病例共43例,比较3种检查方法对肝内及肝外病灶的检出情况。结果:在43例患者中,PET-CT检出了40例患者的127个肝内病灶及22例患者的137个肝外病灶;MRI检出了39例患者的114个肝内病灶及20例患者的103个肝外病灶;螺旋CT检出了35例患者的92个肝内病灶及14例患者的60个肝外病灶。其中PET-CT与螺旋CT比较,总病灶检出数(P<0.05)及肝内肝外病灶检出数(P均<0.001)差异有统计学意义;MRI与螺旋CT比较肝内外病灶检出数差异有统计学意义,P均<0.001;PET-CT与MRI比较中仅肝外病灶的检出数,差异有统计学意义,P<0.001。结论:对肝内外病灶的检出PET-CT与MRI均优于螺旋CT;PET-CT对转移灶的检出较有优势,对高分化肝细胞癌则MRI优于PET-CT;以CT模拟定位确定靶区制定放疗计划时,有针对性的运用MRI和PET-CT的影像资料,具有重要的参考价值。OBJECTIVE: To compare the differences of PET-CT, MRI and spiral CT in detection of primary liver carcinoma lesions so as to accumulate the comparative imaging information for target delineation in radiotherapy on liver cancer. METHODS: The data of 43 cases diagnosed primary liver cancer and inspected by PET-CT, MRI and spiral CT scan before treatment were collected retrospectively, and then the differences of the three methods in detecting intra-hepatic and extra-hepatic lesions were compared. RESULTS: In all the 43 cases, PET-CT detected 127 liver lesions in 40 cases, 22 of which were found 137 extra-hepatic lesions;MRI detected 114 liver lesions in 39 cases, 20 of which were found 103 extra-hepatic lesions. Spiral CT detected 92 hepatic lesions in 34 cases, 14 of which were found 60 extra-hepatic lesions. Statistical analysis: comparisons between PET-CT and spiral CT, total number of lesions (P〈 0.05), extra-hepatic and intra-hepatic lesions ( all P hepati tween d0. 001); comparisons between MRI and spiral CT, extrahepatic and intra-hepatie lesions (all P〈0. 001); comparisons between PET-CT and MRI, extra-hepatic lesions (P 〈0.001) showed statistical significance. CONCLUSIONS, PET-CT shows advantages in detecting lesions. MRI shows the better capacity in detecting well-differentiated hepatocellular carcinoma lesions than PET-CT; they were better in detecting intrahepatic and extrahepatic lesions than spiral CT. It will be useful to refer to the result of lesions detected by PET-CT and MRI while to plan a radiotherapy treatment based on CT-simulation determining target.
