双气囊电子小肠镜对小肠出血的诊断价值

摘　　要：目的评价双气囊电子小肠镜对小肠出血的诊断价值。方法对从2006年1月～2008年2月,我院33例经多项常规检查阴性、疑为小肠出血的患者用双气囊电子小肠镜分别以经口或经肛方式检查完小肠,检查所用平均时间为(50.5±13.4)min;绝大部分患者对检查的耐受性良好。结果检查阳性率87.9%,其中多发性小肠溃疡5例,克罗恩病3例,慢性非特异性炎症2例,小肠间质肿瘤4例,高分化腺癌1例,钩虫病2例,小肠血管瘤出血2例,其他小肠血管畸形出血4例,美克尔憩室2例,十二指肠憩室2例,十二指肠溃疡1例,十二指肠腺癌1例,无明显异常4例。结论双气囊电子小肠镜可直视检查全小肠病变,是一种直观、安全、可靠的检查手段,对不明原因的小肠出血有较高的临床诊断价值。Objective To evaluate value of the double-balloon enteroscopy in the diagnosis of intestinal bleeding. Methods From January 2006 to February 2008,33 cases of hospital wellness checks by a number of negative,the suspected small intestinal bleeding in patients with double-balloon enteroscopy respectively to the mouth or anus by the way the small intestine end inspection,inspection by average time（50.5 ± 13.4）minutes, the vast majority of the examination of patients with good tolerance. Results Inspection positive rate of 87.9%,including multipie small intestinal ulcer five cases ,three cases of Crohn＇s disease,chronic non-specific inflammation two cases,intestinal stromal tumors four cases,one case of well-differentiated adenocareinoma, two cases of hookworm disease,intestinal vascular two cases of bleeding,intestinal vas- cular malformation hemorrhage other four case, two cases of Meckel＇s diverliculum, duodenal diverticulum two cases, one case of duodenal ulcer,duodenal adenocarcinoma one case,no obvious abnormalities four cases. Conclusion Double-balloon enteroscopy can open the entire small bowel lesions inspection is a visual, safe and reliable means of inspection, the small intestinal bleeding of unknown causes higher clinical diagnostic value.
