鼻咽癌咽旁间隙插植三维靶区重建与剂量分布虚拟仿真研究

摘  要:目的通过对实际的咽旁间隙插植与虚拟的插植进行比较,探讨计算机仿真模拟技术应用于鼻咽癌咽旁插植技术的辅助价值。方法对10例鼻咽癌咽旁间隙插植患者设计虚拟理想插植施源管,并对两施源管之间的距离、大体肿瘤靶区和周围重要组织器官的体积剂量直方图进行比较。结果鼻咽咽旁间隙实际插植和虚拟插植施源管之间的距离均数为4.9mm。虚拟插植100%等剂量线所包括的肿瘤靶区体积大于实际插植,而实际插植100%等剂量线所包括的正常组织体积大于虚拟插植。结论运用计算机技术进行咽旁间隙插植模拟有可能减少实际插植与虚拟插植之间的差异,有助于改善肿瘤靶区和周围正常组织器官的剂量分布。Objective To explore the value of clinical application of virtual simulation for determination the dose of brachytherapy in nasopharyngeal neoplasm. Methods Fictitious ideal parapharyngeal space interstitial brachytherapy were designed in ten cases of nasopharyngeal neoplasm. The discrepancy between the real and fictitious program were calculated and the dose volume histograms of gross tumor dose and surrounding critical organs dose were compared. Results The mean value of the distances between the real and fictitious program was 4.9ram. The inside of the 100% isodose line was larger for fictitious implantation, while the volume of body inside of the 100% isodose line was larger for real implantation. Conclusions Virtual simulation can reduce the discrepancy between the real implantations and fictitious ideal implantations and consequently ameliorate the dose distributions of tumor volume and surrounding critical organs.
