子宫内膜间质肉瘤病理观察和免疫组化分析

摘　　要：目的:探讨子宫内膜间质肉瘤的临床、病理诊断、多成分分化的特点及鉴别诊断。方法:分析4例子宫内膜间质肉瘤(3例低度恶性子宫内膜间质肉瘤,1例高度恶性子宫内膜间质肉瘤)临床、病理及免疫表型。结果:3例低度恶性子宫内膜间质肉瘤肿瘤细胞近似于增殖期子宫内膜间质细胞,细胞异型性小。1例高度恶性子宫内膜间质肉瘤出现坏死,肿瘤细胞呈短梭形,核分裂像多。免疫组化标记示肿瘤组织vimentin、desmin、ER、PR阳性,而PCK、EMA、S-100、LCA、CD34均阴性。结论:子宫内膜间质肉瘤发病率低,可伴发多成分分化,诊断依据肿瘤组织浸润生长、核分裂像及免疫组化的结果进行确诊。Objective：To investigate the clinicopathologic features, multi - differentiation, differential diagnosis of endometrial stromal sarcomas. Methods： The clinicopathologic data （ including the histological characteristics, histochemical and immunohistochemical staining features）, complication, differential diagnosis and prognosis of endometrial stromal sarcomas were analyzed. Results：Three cases of lowgrade endometrial stromal sarcoma showed similar morphology with benign endometrial stromal cells and micro anaplasia 1 case of high - grade endometrial stromal sarcoma showed large foci of necrosis, tumor cells were short shape and ten mitotic figures per ten high power fields. Tumors were positive for vimentin, desmin, ER, PR, SMA and negative for PCK, EMA, S - 100, LCA, CD34. Condusion：Endometrial stromal sarcoma is a rare tumor and with multi - differentiation. The diagnosis depends on morphological features and immunohistechemical profile.
