翼状胬肉不同手术方法临床疗效观察

摘　　要：目的比较评价新鲜羊膜移植、自体角膜缘干细胞移植及单纯性翼状胬肉切除或翼状胬肉移位埋藏在治疗翼状胬肉中的临床疗效观察。方法翼状胬肉患者98例,102只眼,分为A、B、C组。A组30眼行单纯翼状胬肉切除或翼状胬肉移位埋藏;B组32眼行翼状胬肉切除联合新鲜羊膜移位术;C组40眼行翼状胬肉切除联合自体角膜缘干细胞移植术。术后随访6～48个月,比较各组翼状胬肉复发率。结果A组30眼中11眼复发,复发率36.37%;B组32眼6眼复发,复发率18.75%;C组40眼复发3眼,复发率7.5%。在原发性翼状胬肉治疗中,三组分别有9眼占36%、4眼占16%、2眼占6,25%复发;在复发性翼状胬肉治疗中,三组分别有2眼占40%、2眼占28,58%、1眼占12.5%复发。结论在防止翼状胬肉复发方面,自体角膜缘干细胞移植可有效地降低翼状胬肉术后的复发率,是目前较理想的手术方法。Objective Relatively evaluate the fresh amnion to transplant, transplant from the body cornea good luck stem cell and pure sex pterygium cut off or pterygium move a bury in cure the pterygium the clinical curative effect observe. Method The pterygium sufferer 98,102 eyes, is divided into the A, B, C group＇s. A 30 eyes of A group go the pure pterygium to cut off or the wing form meat move to bury, 32 eyes of B group go the pterygium to cut off to unite the fresh amnion to move operation, A 40 eyeses go the pterygium to cut off the consociation to do the cell transplantation from the body cornea good luck. After the operation with visit 5-48 months, the each pterygium relapses the rate. Results 11 eyes relapse in 30 eyes of A set, relapsing rate 35.37%, A 32 eyes 6 eyes relapse, relapsing rate 18.75%, A 40 eyes relapse 3 eyes, relapsing rate 7.5%. At the original hair pterygium under medical treatment, it is three to have 9 eyes to have 35%,4 eyes to have 15%,2 eyes to have respectively 5,25% relapse, At relapse sex pterygium under medical treatment, three have 2 eyes to have 40%,2 eyes to have respectively 28,58%,1 eyes has 12. 5% to relapse. Conclusion In the aspects of keeping the pterygium from relapse, after the body cornea good luck stem cell transplanted and can lower the pterygium availably of relapse the rate, is a more ideal surgical operation method currently.
