摘 要:目的:从分子水平研究精神分裂症的发病机制,研究女性精神分裂症患者血清甲状腺激素(FT3、FT4、TSH)水平与临床因素的相关关系。方法:对60例符合CCMD-Ⅲ及ICD-10诊断标准的精神分裂症患者随机分为利醅酮组和阿立哌唑组。用药范围:利醅酮起始剂量1 mg/d^6 mg/d。阿立哌唑起始剂量5 mg/d^30 mg/d,疗程均为8周。在治疗前后检测血清FT3、FT4、TSH水平,分别与健康对照组比较,采用PANSS、CGI、TESS评定精神症状、临床疗效及其他相关因素。结果:研究组治疗前后FT3、FT4水平均高于对照组,并在治疗后呈逐渐下降趋势,而TSH治疗前后均低于对照组。相关性分析显示:治疗前FT4与治疗P因子成正相关(P<0.05),TSH水平与P因子成负相关(P<0.05),病程与FT3呈现正相关(P<0.05)。治疗后FT3与PANSS成正相关(P<0.01)。多元回归分析显示:治疗前病程与FT3成正相关(P<0.05),药物治疗与FT4成负相关(P<0.05),精神分裂症临床分型与TSH成负相关(P<0.05),治疗后PANSS总分与FT3成正相关(P<0.05)。结论:精神分裂症患者存有内分泌的紊乱,患者血清FT3,FT4变化可能与其疾病本身有关,血清FT3、FT4水平受多因素的影响,或许可预测精神分裂症患者的病情的严重程度。Objective:the aim of this study is to study changes of serum thyroxine level in female schizophrenia and its rela- tionship to clinical characteristics, and to study the relationship between Serum and psychopathology, and in order to know the effection on the FT3 ,FT4 ,TSH levels with the antiphychiatric medicine of risperidone and Aripiprazole. Methods : the patients were randomly divided into two groups risperidone and Aripiprazole. Serum FT3 (Free triiodothyronlne) ,FT4 (Free thyroxine) ,TSH( thyroid -stimula-ting hormone) were tested in 60 female patients who fited for CCMD-Ⅲ and ICD - 10 diagnosis of schizophrenia before and after treatment respectively and compared with 30 control subjects, meanwhile PANSS ,CGI,TESS and scales were monitored. Results: Ser- um FT3 ,FT4 levels in patient group before and after treatment were higher than control subjects and decreased after treatment ,but serum TSH is lower than that in control subject before and after treatment. In addition, there was positive correlation between the PANSS scores and serum FT4 scores after treatment( P 〈 0.01 ). there was positive correlation between the P scores and serum FT4 scores before treatment( P 〈 0.05 ), and there was negative correlation between the TSH scores and serum P scores before treatment( P 〈 0.05 ). there was positive correlation between the course of disease and FT3 level before treatment. Conclusions : the findings suggest that there have endocrine confused in schizophrenia patients. The changes of serum FT3 ,FT4 level in schizophrenia patients may be related to ill-ness itself ,serum FT3 ,FT4 level seems to be as predictor for severity of schizophrenia and were effected by many factors.
