摘 要:目的对照研究鼻咽癌MRI和CT成像差异,并探讨其对鼻咽癌T分期(福州92分期)的影响。方法对101例经病理诊断为鼻咽低分化鳞癌的初诊患者,应用1.5TMRI和CT分别进行成像并进行对照研究,在此基础上进一步探讨MRI和CT成像差异对鼻咽癌T分期的影响。结果MRI对病灶发现率在咽后淋巴结(ΔP=-36.6337%)、头长肌(ΔP=-24.7525%)、蝶骨(ΔP=-17.8218%)、枕骨斜坡(ΔP=-25.7426%)、海绵窦(ΔP=-8.9109%),鼻窦(ΔP=-14.8515%)与CT比较差异有统计学意义(P<0.05);按MRI成像T分期为:Ⅰ期1例,Ⅱ期37例,Ⅲ期43例,Ⅳ期20例;按CT成像分期T分期为:Ⅰ期4例,Ⅱ期63例,Ⅲ期25例,Ⅳ期9例。两者构成比比较差异有统计学意义(P=0.000)。结论MRI与CT比较,MRI在鼻咽癌诊断的优势主要表现在发现咽后淋巴结、头长肌、蝶骨、枕骨斜坡、海绵窦和鼻窦的病灶;鼻咽癌MRI分期与CT分期比较,早期(Ⅰ期、Ⅱ期)构成比下降,晚期(Ⅲ期、Ⅳ期)构成比上升。Objective To compare the image of MRI and CT in assessing nasopharyngeal carcinoma (NPC), and evaluate the value of MRI and CT in clinical T-staging of NPC. Methods 1.5 Tesla MRI scans were performed in 101 patients with newly diagnosed poor differentiation nasopharyngeal squarnous carcinoma. The MRI findings were compared to those obtained by CT scans. Comparison of clinical T-stage between those two means were performed. Results MRI was superior to CT in demonstrating lesions in the retropharyngeal node( △P = -36. 6337% ), M. long'us capitis ( △P = - 24. 7525% ), sphenoidal sinus ( △P = - 17.8218% ), clivus ( △P = - 25. 7426% ), cavernous sinus (△P = - 8. 9109% ) and paranasal sinuses ( △P = - 14.8515% ) ( P 〈 0.05 ). The cases distribution of clinical T-stage MRI were : one T1,37 T2,43 T3,20 T4 while those proved by CT were:4 T1,63 T2,23 T3,9 T4. There was a statistic difference in the cases distribution of clinical T-staging proved by MRI and CT( P = 0.000). Conclusion MRI was superior to CT in demonstrating lesions in the retropharyngeal node,sphenoidal sinus,clivus ,cavernous sinus and paranasal sinuses. In T-staging of NPC, the constituent ratio of early stages (TI,T2 ) in MRI was lower than that in CT assessment, while the constituent ratio of terminal stages ( T3 , T4 ) in MRI was higher than that in CT assessment.
