MRI测量导水管和四脑室正常径线及其病理意义

摘　　要：目的：通过正常成人正中矢状面ＭＲＩ三个径线测量，同脑干胶质瘤，脑积水病人相对照，分析其相互关系，阐明临床价值。材料和方法：正常成人１００例，脑干胶质瘤２１例，交通性脑积水２２例，用１．５ＴＭＲＩ机，５ｍｍ层厚，正中矢状面Ｔ１Ｗ图像直接光标测量计算。结果：①从鞍背顶点到小脑幕前缘连线，正常组男５７±４ｍｍ，女５６±４ｍｍ，导水管位于此线起点后，男３０±２ｍｍ，女２９±２ｍｍ；②导水管前后径＜２ｍｍ；③从蝶鞍中心点到小脑幕后下缘附着点连线，男８６±６ｍｍ，女８４±３ｍｍ；四脑室位于此线起点后，男４０±２ｍｍ，女３９±３ｍｍ，脑干胶质瘤组，导水管及四脑室均明显后移（Ｐ＜０．０５），脑积水病人导水管扩张＞３ｍｍ。结论：ＭＲＩ正中矢状面测量不受以往方法的收缩指数及放大系数的影响，无伪影干扰。导水管及四脑室基本位于测量线的中点，便于记忆和应用，为诊断提供了参考数据。Purpose:3 lines of normal adults,brain stem glioma and hydrocephalus were measured retrospectively on middle sagittal plane by MRI to discuss the relationship and the clinical value.Materials and methods:100 normal adults,21 brain stem glioma and 22 communicating hydrocephalus were studied by 1.5 Tesla Superconduct MRI machine with 5mm slice thickness on SE sequence.T 1W image of middle sagittal plane was measured directly on the console.Results:①The distance from the point of the dorsum sellae to the free margin of the tentorium,male was 57±4mm,female was 56±4mm.The aqueduct was placed on the line of the start,male was 30±2mm,female was 29±2mm;②The anterioposterior diameter of the aqueduct was <2mm;③The distance from central point of sella turcica to the lowest posterior point of the tentorium,male was 86±6mm,female was 84±3mm;the fourth ventricle was on the line from the start,male was 40±2mm,female was 39±3mm.The aqueduct and fourth ventricle of the brain stem gliona were displaced backword (P <0.05).The aqueduct of hydrocephalus was enlarged more than 3mm.Conclusion:MRI measurement on the middle sagittal plane was not infuluenced by the indexes of constriction and enlarged ratio of the former methods and was not disturbed by the artifacts.The normal aqueduct and fourth ventricle was placed on the middle point of its line which was convenient and practical use for clinical diagnosis.
