摘 要:目的探讨胸腔积液肺耐药蛋白基因检测临床意义。方法用荧光定量RT-PCR法检测114例肺癌患者胸腔积液标本(腺癌80例,鳞癌17例,差分化癌10例,小细胞癌7例)肺耐药蛋白基因表达,并进行胸腔积液细胞学检测。结果腺癌、鳞癌、差分化癌及小细胞肺癌胸腔积液LRP mRNA阳性率及中位拷贝数分别为72.94%(62/85),45.45%(5/11),40.00%(4/10),28.57%(2/7);23.58,19.22,20.15,8.36 copies/ml。非小细胞肺癌各种病理类型标本中LRP mRNA阳性率均显著高于小细胞肺癌(P均<0.05),在非小细胞肺癌的各种病理类型中,以腺癌LRP mRNA阳性率最高;非小细胞肺癌各种病理类型胸腔积液标本LRP mRNA中位拷贝数无显著性差异(P均>0.05)。结论胸腔积液LRP mRNA检出,不仅明确了肺癌的诊断,也表明该肺癌细胞具有耐药性。Objective To explore the clinical significance of determination of LRP gene assay in pleural fluid of lung cancer. Methods To detect LRP mRNA of 114 pleural effusion samples of lung cancer(80 samples of adenocarcinoma, 17 samples of squamaous cancer, 10 different2al cancer,and 7 samples of small cell lung cancer). In contract,a!l pleural effusion samples of lung cancer were detected by exfoliative cytology. Results The positive detection rate and meso-copies of adenocarcinoma,squamaous cancer,differential cancer and small cell lung cancer discerned 72. 94% (62/85), 45.45%(5/11), 40. 00% (4/10)and 28.57%(2/7);23.58,19.22,20.15 and 8.36 copies/ml. The sensitivity of every type of NSCLC was significantly higher than SCLC (P〈0. 05). The sensitivity of adenocarcinoma was the highest in all types of NSCLC. Conclusion The detection of LRP gene in pleural effusion can not only diagnose that the patient has lung cancer,but also confirm that the lung cancer cells have drug tolerance.
