利培酮口腔崩解片与氯氮平对精神分裂症患者生活质量的影响

摘　　要：目的探讨利培酮口腔崩解片治疗对精神分裂症患者生活质量的影响。方法以利培酮口腔崩解片与氯氮平治疗精神分裂症各40例作对照研究,采用简明精神病评定量表(BPRS)、不良反应量表(TESS)评定疗效及不良反应,WHOQOL-BREF量表衡量患者的生活质量。结果最后一次评定,利培酮口腔崩解片组BPRS总分降至(15.62±4.37),与治疗前比较差异有统计学意义(P<0.01);氯氮平组BPRS总分降至(16.02±4.63),与治疗前比较差异有统计学意义(P<0.01)。两组间疗效差异无统计学意义(P>0.05)。氯氮平组不良反应较利培酮口腔崩解片组多。经12个月治疗,利培酮口腔崩解片组的生理健康、心理健康、社会关系、环境因素的分值分别为71.13±5.63、95.37±11.73、96.31±12.37、95.37±11.92,氯氮平组的生理健康、心理健康、社会关系、环境因素的分值分别为68.31±9.37、78.12±10.34、79.72±12.22、78.65±11.85。两组患者生活质量较治疗前均有显著改善(P<0.01),在心理健康、社会关系和环境因素等三方面,利培酮口腔崩解片优于氯氮平(P<0.01)。结论在提高患者生活质量方面,利培酮口腔崩解片优于氯氮平,有利于精神分裂症的远期康复。Objective To investigate the influence on quality of life of Risperidone orally disintegrating tablets in schizophrenia. Methods 40 patients with schizophxenic were treated with sisperidone as the controlled group. The Brief Psychiatric Rating Scale （BPRS）, while the adverse events were determined by the treatment emergent symptoms scale（TESS） ,WHOQOL-BREF instruments were used to evaluate the patients quality of life before and after drug abministration. Results For the last assessment, The scores of BPRS decrease to 15.62 ± 4. 37 in Risperidone orally disintegrating tablets group, was significantly lower than the beginning（ P 〈 0. 01 ） ; the scores of BPRS decrease to 16.02 ± 4. 63 in clozapine group, was significantly lower than the beyinning（ P 〈 0. 01 ） ; Two groups showed similar effects ,with no significance. The side effects were more in clozapine group than those in guetiapine group. 12 months after treatment, the scores of physical health, psychological health,social relationship, and environment in Risperidone orally disintegrating tablets group were 71.13 ± 5.63,95.37 ± 11.73,96. 31 ± 12. 37,95.37 ± 11.92 ; and the scores of physical health, psychological health, cocial relationship,and environment in clozapine group were 68.31 ± 9. 37,78.12 ± 10. 34,79.72± 12.22, 78. 65 ± 11.85, and the scores of four domains in WHOQOL-BREF instruments had been improved significantly in to groups（ P 〈 0. 01 ）In the damains of Psychological health, social Relation ship and Environment, Risperidone orally disintegrating tablets was superior to clozapine （P 〈 0. 01 ）. Conclusion Risperidone orally disintegrating tablets has more advantage than Cozapine to improve guality of life in schizophrenic and would be helpful to patients longterm rehabilitation.
