上海市DenverⅡ发育筛查量表适应性研究

摘　　要：【目的】上海DenverⅡ发育筛查量表在实践中的应用情况,探讨该量表在儿童保健监测中的意义。【方法】在上海市8个区随机整群抽取1个里委出生至6岁的本市常住儿童。调查人员在调查前进行了统一培训,对每位儿童应用丹佛发育筛查试验(Denver Developmental Screening Test,DDST)、DenverⅡ和Gesell量表同时进行测试,测试结果以Gesell为金标准与DenverⅡ结果比较显示两种方法具有一致性。应用SPSS统计软件进行Kappa检验和Mc-Nemar检验。【结果】共完成260人,受检率99.6%。其中男147人,女113人。DenverⅡ灵敏度为89.5%、特异度为93.2%、假阳性率为6.8%、假阴性率为10.5%、正确诊断指数为0.83、符合率为92.7%。【结论】上海DenverⅡ儿童发育筛查量表用于儿童发育筛查较为合适,可作为儿童保健监测的常规检查,予以推广应用。[Objective] To study applicatition in practice of Shanghai Denver Ⅱ development screening measuring scale and to approach the significance of that measuring scale in monitoring of child care. [Methods] Collectting a cluster of birth to six-year-old city resident children in one neighborhood from the 8 areas of Shanghai city in random. Investigators were all be trained together before the investigation. Application Denver Developmental Screening Test（DDST）,Denver Ⅱ , and Gesell measuring scale every child at the same time. was tested by The Gesell was the gold standard for the testing results and the results indicated the concordance compared with the results of Denver Ⅱ. Kappa analysis and McNemar analysis were used by SPSS statistical software. [Results] All completed 260 children,the examination rate was 99.6%,147 children were boys and 113 children were girls among the total. The sensitiveness of Denver Ⅱ was 89.5 % ,the specificity index was 93.2 %, the false positive rate was 6.8 %, the false negative rate was 10.5 %, the correct diagnosis indexing was 0.83,and the coincidence rate was 92.7%. [Conclusions] Shanghai Denver Ⅱ development screening measuring scale is more applicable for child care screening. It can to be the routine examination for the monitoring of child care and can be used generally.
