摘　　要：目的:探讨鼻咽癌患者血清中细胞角蛋白19片段(CYFRA21-1)及相关细胞因子水平的变化与癌肿发生的关系。方法:30例鼻咽癌患者及30名对照组人员血清CYFRA21-1、IL-2、IL-6、TNF-α及TGF-β含量均采用放射免疫分析;并对两组结果进行比较分析。结果:鼻咽癌患者在同步放化疗前血清上述5项指标中,除IL-2水平极显著低于对照组(P<0.01)外,其余4项指标均较对照组升高极显著(P均<0.01);治疗6个月后患者IL-2水平显著上升,CYFRA21-1水平显著下降,2指标均已恢复至正常人水平,统计显示与对照组比较均无显著差异(P均>0.05);其它3项指标治疗6个月后水平均显著下降,但与对照组比较仍存在显著性差异(P均<0.05)。结论:血清5项指标水平的变化与鼻咽癌关系密切,其测定对该肿瘤的诊断和预后评估有帮助。Objective To investigate the clinical significance of determination of changes of serum contents of CYFRA21 - 1, IL - 2, TNF - α and TGF - β in patients with nasopharangeal carcinoma. Methods Serum levels of CYFRA21 - 1, IL - 2, TNF - α and TGF - β were determined with radioimmunoassay in 30 patients with nasopharangeal carcinoma both before and after treatment as well as in 30 controls. Results Before therapy （ synchronous chemo and radio therapy） serum levels of CYFRA21 - 1, TNF - α and TGF - β were significantly higher （P 〈 0.01 ） in the patients with nasopharangeal carcinoma than those in the controls, while serum IL - 2 levels were significantly lower. After 6 months of therapy, the serum CYFRA21 - 1 and IL - 2 levels were not much different （ P 〉 0. 05 ） in patients from those in controls, however, serum levels of TGF - β, TNF - α and IL - 8 were still significantly higher （ P 〈 0. 05） than those in controls. Conclusion Changes of CYFRA21 - 1, CYFRA21 - 1, IL - 2, TNF - α and TGF - β levels were closely related to the disease process of nasopharangeal carcinoma determination of changes of these markers might be helpful for clini- cal diagnosis and outcome prediction.
