摘 要:目的探讨乳腺癌组织bcl-2、bax基因蛋白表达的临床意义。方法采用免疫组织化学技术、Western blot技术,检测bcl-2、bax蛋白表达情况。结果乳腺腺病、乳腺纤维腺瘤、乳腺癌中bcl-2蛋白阳性率分别为95.24%(20/21)、76.19%(16/21)、61.46%(59/96)),bcl-2蛋白表达在三种疾病之间存在统计学意义(χ2=9.790,P=0.007);Western blot技术检测乳腺腺病、乳腺纤维腺瘤、乳腺原位癌、乳腺浸润性癌中磷酸化bcl-2蛋白表达的平均OD值的比值分别为0.828、0.719、0.524、0.520,结果与免疫组化检测结果一致。随着淋巴结转移数目的增多、临床分期的增高,乳腺癌bcl-2蛋白表达阳性率降低(P<0.05)。乳腺癌中bax蛋白表达阳性率明显高于乳腺良性疾病(χ2=22.822,P=0.000),与淋巴结转移数目及临床分期无统计学意义(P>0.05)。结论bcl-2在乳腺癌中的表达显著性低于乳腺良性疾病,bax在乳腺癌中的表达则显著性高于乳腺良性疾病,二者在判断乳腺良恶性疾病时具有重要辅助价值。bcl-2基因与乳腺癌的发展、预后有一定的相关性。Objective To investigate the expression of bcl -2 and bax, to reveal the correlation between their expression and clinical - pathology parameters of breast cancer and to analyze the effects on the pathogenesis and progression of breast cancer. Methods The data of clinic and pathology with 21 cases of adenosis of breast, 21 eases of fibroadenoma of breast, and 96 cases of breast caner was respectively collected. All specimens were evaluated by histopathology, immunohistoehemistry technique with antibodies against of bcl -2 and bax. The correlationship of these factors was analyzed. 12 cases of every disease were collected. All fresh specimens were evaluated by Western blot technique with antibodies against of bcl - 2. Results The positive rate of bel - 2 protein expression in adeno- sis, fibroadenoma and carcinoma of breast was 95.24% (20/21) ,76.19% ( 16/21 ) and 61.46% (59/96) respectively. There was significant difference between bcl - 2 - positive and bcl - 2 - negative patients in these diseases ( X^2 = 9. 790, P = 0. 007 ). The ratio of the average OD value of the bcl - 2 protein expression evaluated by Western blot technique in adenosis, fibroadenoma, duct cancer in situ and invasive carcinoma of breast were 0. 828,0. 719,0. 524 and 0. 520. The result of bcl - 2 protein expression was evaluated by Western blot technique bax - positive and bax - negative patients from breast carcinoma and benign lesions of breast. There was not sig- nificant difference between bax protein expression with the numbers of lymph glands and clinical stages. Conclusion Bcl - 2 expression in benign lesions of breast is significantly higher than that in breast cancer and bax expression in breast cancer is significantly higher than that in benign lesions of breast. Bcl - 2 expression is associated with the development and prognosisof breast carcinoma.
