急性脑梗死患者血小板表达PECAM-1的动态变化及意义

摘　　要：目的探讨急性脑梗死(AC I)患者血小板表达血小板内皮细胞黏附分子1(PECAM-1)的动态变化规律。方法采用全血流式细胞术检测68例AC I患者(AC I组)不同时间的血小板PECAM-1表达水平,并与35例健康者(对照组)进行比较。结果AC I组血小板PECAM-1表达水平显著高于对照组(P<0.01),48 h达高峰,14 d仍高于对照组(P<0.01);48 h内血小板PECAM-1表达水平与梗死体积呈弱正相关(r=0.39,P<0.05),与神经功能缺损程度无关,14 d时血小板PECAM-1表达水平与神经功能缺损程度呈弱负相关(r=-0.43,P<0.05)。结论血小板PECAM-1参与脑梗死的不同病理生理过程。Objective To investigate the dynamic changes of platelet platelet endotheliocyte cellular adhesion mole-culel （PECAM-1） in patients with acute cerebral infarction （ACI）. Methods The platelet PECAM-1 expression levels of different times in 68 patients with acute cerebral infarction （ACI group）were measured by whole blood flow cytometry, and compared with those of 35 normal controls（control group）. Results The platelet PECAM-1 expression levels in ACI group were higher than those in control group（P 〈0.01 ）, and reached the peak on the 48th hour, and still higher than that of control group on the 14th day（P〈0.01）. The platelet PECAM-1 expression levels had the less positive relation with infarctus volume（ r = 0.39 ,P 〈 0.05）, and had no relation with neurologic impairment degree, and had the less negative relation with the neurologic impairment degree on the 14th day（r=-0.43, P〈0.05）. Conclusions Platelet PECAM-1 partici-pate in the different course of pathophysiology of acute cerebral infarction.
