摘 要:目的探讨慢性粒细胞白血病(CML)患者骨髓单个核细胞染色体核型及血清乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(α-HBDH)变化与其预后的关系。方法采用生化法检测不同病期CML患者的血清LDH、α-HB-DH,取其骨髓有核细胞用R带分析法作细胞染色体分析。结果CML加速期或急变期组复杂核型Ph1以外染色体出现频率及血清LDH、α-HBDH明显高于慢性期组,慢性期组LDH、α-HBDH变化与骨髓幼稚细胞数无相关性;加速期或急变期组二者变化与骨髓幼稚细胞数呈正相关(r=0.699、0.599,P均<0.01)。结论LDH、α-HBDH对CML疾病分期有判断价值,对病情及预后的判断以细胞遗传学检测更准确。Objective To investigate the relationship between the level of serum lactate dehydrogenase ( LDH), α-hydroxy-butyric dehydrogenase(α-HBDH) , the chromosome karyotype of marrow mononuclearcell and the prognosis in the patients with the chronic myelocytic leukemia (CML). Methods The serum concentrations of LDH and α-HBDH in different stages of CML patients were tested by biochemistry method. Marrow karyocytes obtained from the CML patients were detected by using R-banding technique. Results The CML patients in accelerated period or blast crisis stage had not only the higher incidence of the chromosome excluding Ph' but also the higher levels of LDH and α-HBDH than those in the chronic phase. The change of serum LDH and α-HBDH levels in chronic phase group had no correlation with the marrow javenile cell number,but they had positive relation with marrow javenile cell numbers in accelerated period or last crisis group( r = 0. 699,0. 599 ,P all 〈 0.01 ). Conclusions LDH and α-HBDH levels have the judgement value on the staging of CML disease, and cytogenetics detection is more accurate for the judgement of pathogenetic condition and prognosis.
