摘　　要：目的：总结采用经单鼻孔蝶窦入路显微手术切除垂体腺瘤的手术经验,探讨适应证选择、技术要点。方法：采用经单鼻孔蝶窦入路显微手术切除垂体腺瘤治疗患者27例。结果：肿瘤全切15例,次全切除7例,大部切除5例。无手术死亡病例。术中并发脑脊液漏3例、鞍内出血1例,术后并发一过性尿崩症4例,均治愈。无一例并发颅内感染。12例术后2～3个月复查MR I,未见肿瘤复发。结论：经单鼻蝶窦入路显微切除垂体腺瘤,具有手术创伤小、时间短、术后恢复快等优点,是治疗垂体腺瘤安全、有效的方法。Objective：To summarize the operative experience concerning resection of pituitary adenoma by single-nostril transsphenoidal approach.Methods：27 patients with pituitary adenoma were treated with microsurgery via single-nostril transsphenoidal approach.Results：Total resection was completed in 15 patients,subtotal resection in 7,and partial resection in 5,without intracranial infection and death of the patients.Cerebrospinal fluid rhinorrhea was seen in 3 cases and hemorrhage within turkish saddle in 1 case during operation.The postoperative transient diabetes insipidus was occurred in 4 patients.All the complications were healing well,without intracranial infection.The rechecked MRI in 12 patients showed no recurrence of tumor in 2-3 months after operations.Conclusion：The microsurgery through single-nostril transsphenoidal approach was a safe,effective method for treatment of pituitary adenomas,has advantages of minimal trauma,short operative duration and quick postoperative recovery.
