摘 要:目的探讨多药耐药(MDR)基因产物P-糖蛋白(P-gp)、谷胱甘肽-S-转移酶-π(GST-π)及DNA拓扑异构酶Ⅱ(TopoⅡ)在乳腺癌组织中的表达,分析其与临床病理特征及预后的关系。方法采用免疫组织化学方法,检测184例术前未经抗肿瘤治疗的原发性乳腺癌组织中P-gp、GST-π及TopoⅡ基因产物的表达。结果乳腺癌组织中P-gp、GST-π和TopoⅡ阳性表达率分别为49.5%、45.6%和73.9%;2种MDR基因产物共表达率为31.0%,3种MDR基因产物阳性表达率共为67.4%,明显高于单独阳性表达率12.4%(P<0.01)。P-gp、GST-π和TopoⅡ的阳性表达在不同年龄、组织学类型间的表达率差异无统计学意义(P>0.05);在腋淋巴结有无转移者间的表达率差异有统计学意义(P<0.05);P-gp阳性表达率在不同临床分期的差异无统计学意义(P<0.05),GST-π和TopoⅡ的阳性表达率在不同临床分期间有统计学意义(P>0.05)。但三种耐药基因产物阳性表达率在根治术后局部复发和远处转移者明显高于无复发转移者。结论乳腺癌组织中存在着P-gp、GST-π及TopoⅡ表达水平的变化;单基因和多基因协同作用,以多基因共表达为主;阳性表达者根治术后局部复发和远处转移率较高。化疗前联合检测P-gp、GST-π及TopoⅡ对乳腺癌化疗药物的选择具有指导作用,对预后的判断有参考价值。Objective To explore expression of multidrug resistance(MDR)gene production of Glycoprotein(p-gp),P-glutathione-s-transferase-π and TopoⅡ in breast cancer tissues.Methods A total of 184 cases before surgery and non-chemotherapy were detected immunohistochemically for P-gp、GST-π and TopoⅡ.Results The positive expression rate of P-gp、GST-π and TopoⅡwere 49.5%、45.6% and 73.9% respectively.The coexpression rate of 2 MDR gene production and 3 MDR gene production was 31.0% and 67.4% respectively.The positive expression rate of P-gp、GST-π and TopoⅡ were no relation with histological type and age(P〉0.05),and which was significantly associated with lymph node metasis(P〈0.05).The positive expression rate of P-gp was associated with clinical stage.Positive expression rate of GST-π and TopoⅡ were not related with clinical stage but expression positive were significant associated with metastsis.Conclusion The results suggested that expression levels of P-gp、GST-π and TopoⅡwere varied in breast cancer.A combined analysis of expression of MDR before chemotherapy patients with breast cancer might be useful for the selection of drugs in chemotherapy and prognosis of breast cancer.
