摘  要:目的:探讨胃黏膜幽门螺杆菌(helicobactor pylori,H.pylori)感染、环氧合酶-2(COX-2)的表达与胃癌发生、发展的关系.方法:胃癌(GC)标本80例,非典型增生(Dys)40例,萎缩性胃炎(CAG)30例,肠上皮化生(IM)30例,浅表性胃炎(CSG)30例,应用免疫组化方法测定COX-2蛋白的表达,用亚甲兰染色检测H.pylori.结果:CSG,GC中H.pylori阳性和阴性病例COX-2表达差异无统计学意义(P>0.05),但在CAG,IM,Dys中的H.pylori阳性病例中COX-2表达高于H.pylori阴性病例(52.9%vs7.6%,52.6%vs9.0%,67.9%vs33.3%).结论:H.pylori感染诱导COX-2表达发生在胃癌形成的早期阶段.AIM: To investigate the relationship between helicobacter pylori ( H. Fylori ) infection and cyclooxygenase-2 ( COX-2 ) expression in gastric cancer and precancerous lesions, and to study the possible role of H. pylori infection in gastric cancinogenesis. METHODS: In 30 cases of chronic superficial gastritis (CSG), 30 cases of chronic atrophic gastritis( CAG), 30 cases of intestina metaplasia ( IM ), 40 cases of atypical dysplasia ( Dys ) and 80 cases of gastric carcinoma(GC) , the expression of COX-2 protein was detected by immunohistochemistry and H. pylori infection was examined by Methylene Blue staining. RESULTS: In CSG and GC, there was no significant difference in the positivity rate of COX-2 between H. pylori positive and negative patients (P 〉0.05 ). In CAG, IM and Dys, COX-2 protein expression level was increased in H. pylori positive patients when compared with H. pylori negative patients ( 52. 9% vs 7. 6% , 52. 6% vs 9.0% ,67.9% vs 33.3% ). CONCLUSION : The expression of COX-2 is upregulated in the process of gastric cancer. COX-2 may be helpful to the early diagnosis of gastric cancer.
