转移抑制基因KAI1与卵巢癌临床病理因素和预后的关系

摘　　要：目的：探讨卵巢上皮癌组织中KAI1的表达与肿瘤临床分期、转移及预后的关系。方法：采用逆转录聚合酶链反应（1it-PCR）和免疫组化（S-P）法对45例具有完整临床及随访资料的卵巢上皮癌组织进行KAI1标记检测，并分析其与临床病理因素和预后的关系。结果：KAI1基因表达与肿瘤体积、患者年龄无关，与肿瘤的临床分期、淋巴结转移及复发均呈明显的相关关系。I～Ⅱ期表达显著高于Ⅲ～Ⅳ期（P〈0．05），淋巴结未转移组KAI1表达率高于转移组，有非常显著意义（P〈0．01）。未复发组强阳性表达率为40．74％，显著高于复发组（P〈0．01）。高分化组强阳性表达率高于中分化组和低分化组（P〈0．05）。结论：KAI1基因调节肿瘤的侵袭和转移，其在卵巢上皮癌中的表达，可作为估测肿瘤转移和预后的重要指标之一。Objective： To detect the expression of KAI1 in ovarian epithelial carcinoma （OEC） with the relationship of the clinical stage, metastasis and prognosis. Methods-The expression of KAI1 was analyzed by reverse transcription polymerase chain reaction （ RT - PCR） and immunohistochemistry technique in 45 eases of OEC. The relationship between the results and clinicopathological features were studied. Results： There were no relationship between expression of KAI1 and ages of patients, tumor volume and histological types of OEC. The expression of KAI1 was correlated with clinical stage, lymphnode metastasis and recurrence. KAI1 expression in stage I～Ⅱ was higher than that in stage Ⅲ～IV （ p 〈 0. 05 ）. KAI1 expression in tumor of lymphnode - negative patients was higher than that in lympknode - positive patients （P 〈 0. 01 ）. The strong positive rate was higher in nonreeurrenee patients than that in recurrence patients （ P 〈 0 01 ）. There were significant difference in strong positive rate between high and moderate, low histological grades （ P 〈 0. 05） . Conclusion： Gene KAI1 is essential for the control of invasion and metastasis. The expression of KAI1 in OEC is helpful for assessing the metastasis and prognosis.
