南阳地区乳腺癌422例临床病理研究

摘　　要：目的:了解乳腺恶性肿瘤在河南南阳市区的发病规律和特性。方法:采用SP免疫组化法检测ER,PR,C-erbB-2,p53,PS2,MDR-1在乳腺癌中的表达,并对近5年来422例乳腺恶性肿瘤病例进行统计分析。结果:乳腺恶性肿瘤构成比约为40%,但乳腺癌的外检例数增加了2倍;发病年龄最小22岁,最大81岁,以41岁-50岁组比例最高。年龄<40岁病例占25.6%,表明乳腺恶性肿瘤发病有年轻化趋势。肿瘤类型以浸润性导管癌为主(81.37%),其次是导管原位癌;C-erbB-2表达与ER受体表达呈负相关,ER(-)患者C-erbB-2表达率高,ER(+)患者C-erbB-2表达率低。癌基因C-erbB-2阳性病人,淋巴结转移率较高,C-erbB-2阴性病人淋巴结转移率低,二者有明显差异。结论:南阳地区乳腺癌发病率明显高于国内外研究结果,治疗预后有关的癌基因表达情况与国内外报道基本一致,p53表达偏低,MDR-1表达偏高。Objective： To investigate the pathogenetic features of breast malignant tumors in south - east （ Nanyang city） , Henan. Methods： The expressions of ER, PR, C- erb B-2, p53, PS2, MDR- 1 were examined in breast malignant tumors with immunohistochemistry on 422 eases in recent five years of breast malignant tumors. Results ： The proportion of breast malignant tumors between 2002 and 2006 was approximately 40% , but the number of biopsy increased by 100% ;the youngest age was 22, the oldest age was 81 ,the propotion below 40 was 25.6%, it means that there is a young tendency in breast malignant tumors. The tumor type was dominated with invasive ductal carcinoma（81.37% ） , the second was DCIS, the expression of C -erb B -2 showed a negative correlation with the expression of ER, the expressive rate of C - erb B -2 in RE（ - ） was higher, while the expressive of C - erb B -2 in RE （ ＋ ） was lower; the metastasis of lymph node was higher. Conclusion： Breast cancer incidence in nanyang is higher than other area around world. Treatment prognosis of correlative cancer gene expression is accordance with the literature results at home and abroad, p53 is lower,while MDR - 1 is higher.
