螺旋CT三期扫描在胃癌术前评估及分期中的应用价值

摘　　要：目的评价低张力口服水螺旋CT三期扫描对胃癌的诊断、分期及可切除性判断的价值。方法65例患者口服水，在低张状态下进行动态多期增强扫描。判断肿瘤诊断及分期，预测手术可切除性，并与手术及病理相对照。结果CT表现为胃壁增厚、软组织肿块65例，6例早期胃癌在动脉期及门静脉期明显强化，在平衡期强化程度明显下降，48例进展期胃癌中，42例以门静脉期强化程度为著。本组病例54例行手术治疗，11例螺旋CT评估为Ⅲ、Ⅳ期放弃手术而行介入和化疗。CT分期与手术病理分期相符者44例，胃癌CT分期总的准确率81.5％。结论低张力口服水螺旋CT多期扫描在胃癌诊断、分期及可切除性判断等方面具较高的价值，能够为临床诊治提供有益的指导。Objective To evaluate the value of triple phase dynamic Helical CT with water distention and induced hypotonia in the preoperative diagnosis, staging and assessment of resectability of gastric carcinoma. Methods Sixty five patients with gastric carcinoma underwent triple phase dynamic enhanceed Helical CT. Preoperative CT diagnosis, staging and assessment of resectability were made and correlated with that of operation and pathology. Results CT signs of gastric carcinoma demonstrated thickened gastric wall and soft tissue mass in Sixty-five case, Six case of early stomach cancer showed marked enhancement in the arterial and portal venous phase and obvious attenuation of enhancement in the equilibrium phase.On the contrary,42of 48 advanced gastric carcinoma was showed marked enhancement in the venous phase compared with the arterial phase. Fifty four patients underwent operation while the other 11 with stage III and IV disease underwent interventional and chemical therapy. The overall accuracy of tumor staging was 81.5 for Helical CT with 44cases were correctly staged. Conclusion Triple Phase Helical CT is of high value in diagnosis, staging and assessment of respectability of gastric carcinoma.
