摘 要:目的:检测国人散发性乳腺癌病变中BRCA2和P53的异常表达,并探讨其相关性。方法:利用免疫组织化学技术,检测32例乳腺增生病、28例乳腺癌癌旁组织及60例乳腺癌中BRCA2和P53的表达。结果:①BRCA2在乳腺增生病、乳腺癌癌旁组织、乳腺癌中的阳性表达率依次递减,在三者中的表达有显著性差异(P<0.05);②随着乳腺癌组织学分级的升高,BRCA2基因表达缺失增多,BRCA2的表达与组织学分级呈负相关(r=-0.306,P=0.021)。BRCA2在乳腺癌无淋巴结转移组、淋巴结转移≤4枚组及淋巴结转移>4枚的病例组的阳性表达率统计学处理无显著性差异(P>0.05),但可以显示出随着乳腺癌淋巴结转移数目的增多,BRCA2的表达呈下降趋势;③BRCA2的表达与年龄、TNM分期无明显关系;④P53在乳腺增生病、乳腺癌癌旁组织、乳腺癌中的阳性表达率依次递增,在三者中的表达有显著性差异(P<0.05)。P53的表达与TNM分期呈正相关(r=0.344P=0.019),与组织学分级、淋巴结转移及年龄无明显关系;⑤乳腺癌组织中BRCA2与P53表达之间无显著性相关(r=-0.074,P=0.570),但可显示出随着P53阳性表达的增强,BRCA2的表达呈下降趋势。结论:乳腺上皮中BRCA2的表达减少,P53表达增多对乳腺癌发生发展具有重要作用。对BRCA2及P53的检测有利于筛选出乳腺癌高危人群,早期发现乳腺癌。但BRCA2和P53在乳腺癌中的表达无相关性。Objective:To investigate the significance of the expression of breast cancer susceptibility gene 2 (BRCA2) and P53 in sporadic breast cancer tissue and its significance, and to clarify the correlation of the expression of BRCA2 and P53.Methods: The expressions of BRCA2 and P53 protein in 32 cases of cyclo- mastopathy, 28 cases of para - cancerous, 60 cases of breast carcinoma were examined by immunohistochemical SP method. Results: (1)The positive expression rate of BRCA2 decreased in turn in cyclomastopathy, para- can- cerous and breast carcinoma, there was a significant difference among the groups(P 〈 0.05). (2)The expression of BRCA2 was reversely correlated with histological grade. The lower rate of the expression of BRCA2 was found in those patients with higher histological grade ( r = - 0. 306, P = 0. 021) .BRCA2 expression also reduced in breast cancers with lymph node metastasis increasing. (3)BRCA2 was not correlated with age of onset and TNM stage. (4)The positive expression rate of P53 increased in turn in cyclomastopathy, para - cancerous and breast carcinoma, there was a significant difference among the groups(P 〈 0.05). The positive expression rate of P53 was correlated with TNM( r = 0. 344, P = 0. 019 ). This positive expression rate in stage Ⅲ of breast cancer was higher than that in Stage Ⅰ (P 〈 0.01)and was also higher than that in Stage Ⅱ (P 〈 0.05 ). The expression of P53 was. not correlated with histological grade, lymph node status and age(P 〉0.05). OThe positive expression rate of BRCA2 decreased with the level of P53 expression increasing.But there were not significant correlations between them( r = - 0. 074, P = 0,570). Conclusions: The results suggest that the reduction of BRCA2 expression and the increasing of P53 expression play an important role in the occurrence and development of breast cancer.
