血清白介素-1、白介素-6、肿瘤坏死因子-α与慢性肝病的相关性研究

摘　　要：目的:探讨白介素-1(IL-1)、白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)在慢性肝病患者血清中含量变化的临床意义。方法:40例健康组、40例慢性肝炎组、40例肝硬化组,且肝硬化组按Ch ild-pugh分级分为A、B、C级,用放免法检测血清中IL-1、IL-6、TNF-α、透明脂酸(HA)、层连蛋白(LN)、Ⅲ型前胶原(PC-Ⅲ)、Ⅳ型前胶原(C-Ⅳ)的含量。结果:慢性肝炎组血清IL-1、IL-6、TNF-α、HA、LN、PC-Ⅲ、C-Ⅳ含量高于正常对照组(P<0.05),肝硬化组高于正常对照组(P<0.01),肝硬化组高于慢性肝炎组(P<0.05);肝硬化各组比较,B级高于A级(P<0.05),C级高于A级(P<0.01),C级高于B级(P<0.05);IL-1、IL-6、TNF-α与HA、LN、PC-Ⅲ、C-Ⅳ间呈正相关(P<0.05)。结论:IL-1、IL-6、TNF-α是肝纤维化非创伤性诊断的血清学指标之一,可判断肝病发展的程度,可作为临床动态观察慢性肝病严重程度及判断预后的指标,也为慢性肝病肝纤维化的治疗提供新的思路。Objective： To discuss the effects of IL - 1, IL - 6, TNF - α on chronic liver disease. Methods ： The surum level of IL - 1, IL - 6, TNF - α, HA, LN, PC -Ⅲ, C - Ⅳ of 40 chronic hepatitis and 40 cirhosis patients and 40 normal control individuals were detected with radio- immunity method,and cirrhosis patients were divided three grads（ A,B, C） according to the Child -pugh classification. Results ： IL- 1, IL- 6 ,TNF-α, HA, LN, PC - Ⅲ, C - Ⅳ surum level of liver fibrosis in three groups were gradually increased with aggravation. IL - 1 ,IL - 6, TNF - aserumal level of inside of cirrhosis group were gradually increased along with hepatic function aggravation. IL - 1, IL - 6, TNF - α were positive correlated with HA, LN, PC - Ⅲ, c - Ⅳ. Conclusion ： The serumal IL - 1 ,IL -6 ,TNF -α paticipated in the development of chronic liver fibrosis and might be considered as the serum standards of diagnosing and assessing severity degree of chronic liver disease, as well as it can offer new ideas for treating liver disease.
