老年人胃癌根治术全麻苏醒期血气监测对术后并发症的预防作用

摘　　要：目的:探讨老年人胃癌根治术全麻苏醒期的血气变化及术后并发症的预防作用。方法:选择ASAⅠ～Ⅱ级胃癌根治术病人,65岁≤年龄<65岁各60例。依不检测或检测血气分Ⅰ或Ⅱ、Ⅲ或Ⅳ组每组30例。按血气测定值对症治疗。结果:Ⅱ与Ⅳ组的血气值差异显著(P<0.05或P<0.01),Ⅱ组经血气值指导治疗后,拔管时间缩短、苏醒质量提高,术后并发症减少,与Ⅰ组差异显著性(P<0.05或P<0.01)。结论:老年病人胃癌根治手术,全麻苏醒期、自主呼吸恢复、停呼吸机、吸空气5 min后,血气监测发现多项指标异常,根据结果指导临床治疗,对缩短拔管时间,提高苏醒质量,减少术后精神障碍,呼吸循环等并发症有重要意义。Objective：To investigate the blood gas change of analepsia period of general anesthesia in elderly patients after gastric cancer surgery and the preventive role of guidance treatment of blood gas monitoring on postoperative complications.Methods：The patients with ASA Ⅰ-Ⅱ gastric cancer aged ≥65 years old or≤64 years old were selected with 60 eases in each of them,and divided into group Ⅰ or Ⅱ and group Ⅲ or Ⅳ according to the blood gas analysis being monitored or not,with 30 eases in each group.The symptomatic therapy was used in accordance with the determination values of blood gas.Results：There was significant difference in blood gas values between group Ⅱ and group Ⅳ（P〈0.05 or P〈0.01）.Compared with group Ⅰ after guiding treatment,group Ⅱ had significant difference in blood gas values, shortening the extubation time, improving the quality of analepsia and decreasing the postoperative eimplieations （P〈0.05 or P〈0.01）.Conclusion ：Blood gas monitoring showed that the multiple indices were abnormal during the analepsia period of gen- eral anesthesia （spontaneous respiration recovery ,stopped breathing roaching and air inspiration later） in the elderly patients after gastric cancer surgery.Guiding the clinical treatment based on the results was very important to shorten the extubation time,improve the quality of analepsia ,and reduce the postoperative mental disorder and the respiratory circulation relevant diseases.
