摘 要:目的探讨弥漫性大B细胞淋巴瘤(DLBCL)组织中核转录因子(NF-κBp65)、血管内皮生长因子(VEGF)的表达及其与微血管密度(MVD)的关系及临床意义。方法应用免疫组化法对58例DLBCL组织和10例反应增生淋巴结组织中NF-κBp65、VEGF和MVD进行检测。结果58例弥漫性大B细胞淋巴瘤组织中NF-κBp65阳性表达率为58.8%,VEGF阳性表达率为55.2%,两者在反应增生淋巴结组织中都不表达。NF-κBp65的表达与VEGF的表达呈正相关。NF-κBp65与VEGF阳性表达组MVD为35.63±10.23、34.24±10.54,高于NF-κBp65、VEGF阴性组的29.12±6.24、27.32±5.86(P﹤0.05)。NF-κBp65阳性表达与临床分期、出现全身症状、疗效及总生存期短显著相关(P﹤0.05),VEGF阳性表达与临床分期、疗效、IPI、LDH及总生存期短显著相关(P﹤0.05)。结论NF-κBp65信号通道促进VEGF高表达,影响肿瘤内MVD,与弥漫性大B细胞淋巴瘤的发生、发展关系密切。Objective To study the expression of NF-κBp65, VEGF and its correlation with mierovaseular density(MVD) in diffuse large B -cell lymphoma (DLBCL) and to evaluate their clinical significance. Methods The expression of NF-κBp65, VEGF and MVD were detected in 58 cases of DLBCL and 10 cases of reactive proliferative lymph node by immunohistochemical technique. Results The positive rate of NF-κBp65 in DLBCL was 58.8% and that of VEGF was 55.2%, but none of them was expressed in reactive proliferative lymph node. The expression of NF-κBp65 was significantly associated with the stage, B syndromes, curative effect, IPI and short survival time (P 〈 0.05). At the same time, the expression of VEGF was greatly connected with the stage, curative effect, IPI, LDH and short survival time (P 〈 0.05). NF- κBp65 expression was positively related to the expression of VEGF in DLBCL. The mean MVD value in group with NF- κBp65 and VEGF positive expressions was positively significantly than that in the negative group (P 〈 0.05). Conclusions The activation of NF-κBp65 contributes to MVD in DLBCL by up-regulation of VEGF, which is closely related to the tumorgenesis and development of DLBCL.
