嗅鞘细胞脑内移植治疗持续性植物生存状态5例

摘  要:北京康复中心神经外科2005/2006年对5例持续性植物生存状态患者行人胚胎嗅鞘细胞移植。人胚胎嗅鞘细胞为流产胚胎培养,获赠者知情同意。5例患者病程8~60个月,2例曾接受分流手术,5例接受系统康复治疗,均无效。在局麻下于患者双侧额叶放射冠位置分别注射人胚胎嗅鞘细胞约100万个,术后无任何并发症和不良反应。移植后3周,采用持续植物状态评估量表评价神经功能,与术前比较4例分数升高,神经功能有所改善,1例无变化;4例予以残疾等级量表评估,3例分数下降残疾程度育所减轻,1例无变化;3例进行ISONOVS植物状态量表评估,2例分数升高植物状态有所好转,1例无变化。Five patients with persistent vegetative state (PVS) underwent human fetal olfactory ensheathing cell (OEC) transplantation in Department of Neurosurgery, Beijing Rehabilitation Center from 2005 to 2006. The human fetal olfactory ensheathing cells were derived from abortion, and the informed consent was obtained from the family. Five cases sustained the disease for 8-60 months, two cases underwent the procedure of ventriculoperitoneal shunt and five received rehabilitative treatment. Under local anesthesia, the surgical transplantation of OEC procedure was performed. About 1 X 106 cells were injected into the bilateral frontal lobe of the patient's brain. No complications or adverse effects were observed within three weeks post-transplantation. The neurological function was evaluated by using the Persistent Vegetative State Rating Scales three weeks after the transplantation; 4 cases were improved compared with before operation, and 1 case did not change. Four patients were evaluated using Disability Rating Scale; the scores of 3 decreased, indicating a relieved degree, and 1 case did not change. Three patients were also evaluated by using the ISONO VS scales, in which the scores of 2 cases increased, indicating a improvement, and 1 case did not change.
