改良Millard法同期鼻畸形整复矫治双侧唇裂46例

摘　　要：目的:探讨双侧唇裂和鼻畸形同期整复的手术要点。方法:随机选取46例双侧唇裂(伴/不伴腭裂)患者,应用改良Millard法进行唇裂整复,术中解剖复位口轮匝肌,重建口轮匝肌环;恢复干湿唇交界线的连续性,重建唇珠形态。同期初步矫正鼻畸形,延长鼻小柱。结果:患儿术后上唇瘢痕近似于人中嵴;唇红缘、干湿唇交界线连续、协调、对称,唇红丰满,上下唇、鼻底宽度协调,鼻小柱延长明显,鼻尖形态满意。结论:改良后Millard法可一期完成双侧唇裂和鼻畸形整复。术中便于同期行鼻畸形早期整复,能有效地初步纠正鼻畸形,手术效果理想。Objective To explore the main points of one stage surgical repair for bilateral cleft lip and nasal deformity. Methods 46 patients with bilateral cleft lip （with/without cleft palate） were selected randomly and were assigned to receive modified Millard technique. The orbicularis oris was anatomically reset, the orbicularis oris ring was rebuilt, the continuity of the red line and the perfect vermilion of the lip was reconstructed. At the same time, the nasal deformity was primarily repaired, and the nasal columella was lengthened. Results Of all the patients, there were ideal effects such as symmetric peak of Cupid＇s bow, obvious philtrum notch, full vermilion of the lip, little scarring, satisfactory contour, and bilateral upper lip was symmetric, coordinated, and balanced. The width of nasal alar base was balanced and the columella was obviously lengthened with satisfactory appearance of the nasal tip. Conclusion Modified Millard technique could be applied to repair bilateral cleft lip and nasal deformity at one stage and the effects of the surgery were satisfactory.
